On 9/19/19 at 1:50 P.M., Licensing Program Analyst (LPA) Van met with teacher, Hu Mei Yi, for an unannounced annual random inspection. The purpose of the inspection was explained, and was granted entry to the facility by the teacher. This is a Title 5 Preschool program. Present today are 35 children with 6 teachers. Staff to children ratio are met. LPA observed that facility has an electronic sign in / out record with full legal signatures from parent or authorized representative. Days and hours of operations are from Monday - Friday 7:30 AM - 5:30 PM.

With teacher, LPA inspected the day care areas for health and safety hazards. The facility is in good repair, free of any hazards, and has proper temperature and ventilation. Variety of age appropriate toys and learning materials for children are available. Classroom is set up for child care; furniture, equipment, sufficient amount of sleeping mats for children are available. All children's chairs and table have rounded corner. There are no pools, spas or other bodies of water at the facility. All bathrooms, toilets and sinks are clean and well sanitized. Per teacher, there are no firearms or weapons at the center. Facility has smoke detector, carbon monoxide detector, fully charged fire extinguisher and a working telephone at the site. All cleaning solutions, poisons and other chemicals that are dangerous stored inaccessible to children. Sick children will be separated from the group and will be waiting in the front desk location for parents to pick up.

LPA observed the unused electrical outlets have covers installed. Facility has water fountain and water bottles brought from home readily available for children. Per teacher, This center provides Incidental Medical Service (IMS), however, this current school year, facility does not have any child on IMS plan. Per teacher, the center provides snacks, breakfast, and lunch for children. Teacher stated food are prepared and delivered from chefables. LPA observed the food preparation area is clean, organize, and foods store appropriately.

This facility has no outdoor space and was granted an outdoor waiver on November 6, 2006. The facility uses Willie Wong Chinese Play Ground (850 Sacramento st) for outdoor play space. However, due to the renovation of Willie Wong Chinese Playground, Director requested to previous LPA back in November 2018 to temporary uses St Mary's Square as an outdoor play space. LPA reminded teacher to ensure that playground is cleared of any debris or harmful material before each use.

LPA reviewed the staff and children records. Children's files were reviewed and found to have medical assessments, needs/services, and signed admission agreements. Staff files are organized, complete, and have documentation of education credits. LPA observed multiple staff members have record of valid CPR card in file. There are menus posted at least one week in advance and are visible to the child's authorized representative. LPA also observed facility has posted the required forms (i.e. License, menus, waivers, Notification of Parent's Rights, Notification of Personal Rights, Car Seat Law, and Emergency Disaster Plan). Teacher states Fire and Earthquake drills are done once every two months, the last drill was logged on 06/04/19.

LPA reminded the teacher that as of September 1, 2016, a person may not be employed or volunteer at a child care facility unless he or she has been immunized against influenza, pertussis, and measles. LPA observed the facility has record of required immunization of all staff.

LPA reviewed AB 1207 with the teacher. As of January 1, 2018, all staff is required to complete Mandated Reporter Training every two years. The training can be obtained online at www.mandatedreporterca.com. LPA observed the training completion certificates of staff in file.

No deficiencies are cited today. The copy of this report is reviewed and provided to the teacher. Notice of site visit is posted and shall remain posted for next 30 days.
