LPA observed the outdoor play area to be free of debris and other loose articles. There is a play structure that is intended for use by children 2-5 years of age. The play structure is cushioned with resilient padding in the form of rubber mats. The outdoor play area is enclosed by a fence that is at least four feet high. There are age-appropriate toys and equipment present. There are no pools or other bodies of water present in the facility.
LPA reviewed five teacher’s files, five children’s files, and facility records. There is at least one staff member present with current Pediatric First Aid/CPR training. All staff members present are fully qualified teachers. Immunization records are available for review. Teachers are employees of the San Francisco Unified School District and as such conduct criminal background checks through the school district. LPA advised that all staff must complete Mandated Reporter Training pursuant to AB1207. LPA observed all children’s files to be complete. All required postings are accessible and available for review in each classroom. The facility tested its fixtures for lead and had an exceedance in two fixtures. LPA observed that the fixtures, a water fountain in the outdoor play area and a fountain in Room 105A, have been capped and rendered inaccessible to children in care.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a
maximum of 30 days per person will be assessed if this regulation is violated.
