25 | On April 10, 2025 at approximately 9:30am, Licensing Program Analysts (LPA) Melissa Zaragoza conducted an unannounced, case management visit to SFUSD Junipero Serra Early Education School (PS) . LPA met with principal/facility representative, Danielle Loughridge, and explained the purpose of the visit. Present during LPA's visit included 9 teaching staff and 49 children present. Facility is operating within capacity limits and ratio during LPA's visit. Teaching staff present have fingerprint clearance through the school district. Childcare center hours of operation are Monday through Friday, from 7:30am-5:30pm.
LPA conducted a visit in regards to an unusual incident's report that was reported to the department. The unusual incident was reported on April 4, 2025. The incident involves Child C1, Staff S1, and S2. On April 2, 2025 at around 12:00pm. Per S1, they were leaving the main office, when they heard yelling, and they walked over to Room 3.
Once arrived at the classroom, per S1, they saw S2 tell C1, “don’t do that”. Per S1, S2 was observed to grab C1’s arm and tug. S1 walked over to S2 and S2 said C1 was going to wash their hands. Per S1 they asked C1 if they wanted to go for a walk. Per S1, C1 said, “no”, and went to wash their hands. S1 spoke with S2 and asked about the incident. Per S1, S2 stated, C1 was trying to leave the class to give their sibling a hug, and they did not want C1 to leave out of the class. Per S1 they observed C1's arm the next day for any bruising, or redness, and arm had no marks.
Per S1, they tried to reach out the C1’s guardian via email, phone call, and in person. Per S1, they will try to contact C1’s guardian via mail, to inform guardian of the incident. Per S1, they had a meeting with S2. Per S1, there is always three staff members assigned in the classrooms. Per S1 classrooms, include the lead teacher, and 2 para educators. Per S1 as a plan of correction, they will have 3 para educator present in the classroom when permitted. Per S1, they will also have their positive behavior intervention specialist conduct visits constantly, and conduct a, Adult, Student Boundaries Training, and have a Mental Health Consultant for staff in the classroom.
S1 will inform LPA of any further updates, and pertinent information, on the Incident Report.
