25 | On May 05, 2023, at approximately 1:00pm, Licensing Program Analyst (LPA) Ly arrived at facility to conduct an Unannounced Required - 1 Year Inspection and met with Site Director, Cathy Inamasu and Site Supervisor Nicholas Li. Purpose of the visit was explained. This is a School-age program located in the Christ United Presbyterian Church. Facility's Days and hours of operation: Monday-Friday 2:00PM-6:00PM. Children arrived at facility at 2:20pm. Present during the visit are Site Director, Site Supervisor and 5 staff caring for 49 School Age Children.
LPA and Site Supervisor conducted and inspection of the school age areas for Health and Safety Hazards.. The area that the school age children are using are the church's social hall and the yard. There are no bodies of water present at the center. Per Site Director, there are no firearms or weapons. Disinfectants, cleaning solutions and other potentially harmful items were inaccessible to the children. Currently no children on site need on going medication. The furniture and equipment are in good condition, free of sharp, loose or pointed parts. Children are provided snacks while they are at the facility. Drinking water is available for the children as well as children having their own water bottles. The facility is equipped with a fully charge fire extinguisher, smoke detectors and carbon monoxide detectors. Females restrooms and males’ restroom are separated from each other. All restrooms were in safe and sanitary operating condition. LPA and Site Director also toured the outdoor play area. LPA observed outdoor equipment is in good condition. The play structure appears to be age appropriate for children in care. Areas under slides and other climbing equipment is adequately cushioned.
Children are being signed in/out of the program with clipboard by the entrance. Last Emergency Drill was conducted on 04/07/2023 and is properly logged. First Aid Kit and Emergency supplies are properly maintained and kept inaccessible to children. Cleaning products and Toxins are maintained inaccessible to children.
On this day, a random sample of children's files and found to have emergency identification information and parent's rights. Staff files were also reviewed on this day. Staff files are found to be complete. At least one staff have current Pediatric First Aid and CPR. Facility has posted all the required licensing forms in a prominent accessible location.
