On May 23, 2024., Licensing Program Analysts (LPAs) Van and Basak met with the office manager, Shirley Nhan, who granted LPA entry to the Center. Six teachers were supervising 26 children. During the visit, the Director, Erica Almaguer, arrived. The purpose of the inspection is to follow up on an unusual incident and discuss preventative measures to ensure the safety and well-being of all children at the Center. On May 14, 2024, the Center reported an incident involving the violation of a child's (C1) personal rights.

According to the interview and incident report, teacher S2 witnessed child C1 crying and hitting teacher S1. Upon further investigation, it was revealed that Teacher S1 had flicked Child C1's ear, which led to Child C1's reaction by hitting Teacher S1. Teacher S1 also admitted to the action. As a result, the following Title 22 deficiency was cited on the next page.

After the incident, S1 has been dismissed. The Center will take proactive measures to address the issue in the next two weeks, including a comprehensive review and training on children's personal rights. All training materials and the teachers' signatures confirming their attendance will be submitted to the Licensing office.

A notice of site visit was given and must remain posted for 30 days.

An exit interview was conducted, and the report and appeal rights were reviewed with the Director, Erica Almaguer.

The Center Director will ensure that all staff receive training within the next two weeks, which will include a thorough review and training session on children's personal rights. All training materials and the teachers' signatures confirming their attendance will then be submitted to the Licensing office no later than June 6, 2024.

This requirement was not met, as evidenced by the file reviewed and the interview. Teacher S1 had flicked Child C1's ear, causing Child C1 to react by hitting Teacher S1. This poses a potential health and safety risk to children in care.
