On 3/23/2023 at 9:25AM., Licensing Program Analyst (LPA), Luis J. Gomez met with Co-education Director, Mechele Pruitt. Purpose of inspection was explained and was for an unannounced, complaint investigation. Present was the Co-education Director and 12 staff, 74 children. LPA inspected facility for health and safety hazards.

During today's inspection, LPA performed site observations, interviews with staff/ co-education director, and reviewed facility records. At 12:55PM., Based on interviews and observations, LPA confirmed staff member, S1, asleep in starfish classroom during program’s nap time. Deficiencies were cited on attached Case Management Report (LIC809).

During the course of the investigation, site observations were conducted on 2/1/2023, 2/15/2023 and 3/23/2023. A review of the facility records was completed, which included the Parent handbook; Staff Records; Children's Records; and Incident Report Log. LPA conducted interview with the Co-Eduation Director, Staff, Children and involved parties. (REFER TO LIC9099C FOR CONT.)

Based on evidence collected, LPA was unable to determine if child sustained unexplained bruises/scratches while in care. During inspection, LPA observed classrooms operating within the required teacher-child ratio.

Based on evidence collected, LPA was unable to determine if staff do not ensure incident reports are provided to responsible parties. During interviews, staff stated all incidents are reported to child’s authorized representative and documented.

Based on evidence collected, LPA is unable to determine if staff used physical forms of punishment with child in care. Parent handbook states, teachers use appropriate intervention techniques when assisting children with behavioral needs.

Based on evidence collected, LPA in unable to determine if staff did not prevent child from having an altercation with another child in care. During interviews, staff stated de-escalation strategies are used when assisting children.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is Unsubstantiated.

LPA conducted exit interview with Co-education director, Mechele Pruitt. Complaint report explained and the Notice of Site Visit was posted during inspection.
