25 | On April 06, 2023, at approximately 2:45pm, Licensing Program Analyst (LPA) Ly arrived at facility to conduct an Unannounced Required - 1 Year Inspection. When LPA arrived, LPA was lead to the cafeteria but no one was present at the time as program does not begin until 3:45pm. LPA sat infront of the cafeteria and wait for program staff. Staff and Director arrived at facility at 3:15pm. LPA met with Director, Mimi Sarkisian. Purpose of the visit was explained. Program operates M,T, Th,F after-school, 3:45pm-6:00pm during the School year. For Wednesday program operates from 2:30pm-6pm. Present during the visit is Site Director and 4 staff caring for 40 School Age children.
LPA and Site Director conducted and inspection of the school age activity areas for Health and Safety Hazards. Per Director due to low enrollment this school year, after school program is only being held in the cafeteria. The cafeteria is clean and orderly. There are no bodies of water present at the center. Per Site Director, there are no firearms or weapons. Disinfectants, cleaning solutions and other potentially harmful items were inaccessible to the children. Medications are stored out of reach of the children. The furniture and equipment are in good condition, free of sharp, loose or pointed parts. The kitchen/food prep area was clean and free from litter or debris. All food was stored appropriately. All solid waste storage containers have secure, tight fitting lids. Program offer snacks to the children. Children are observed to have their own water bottles. The cafeteria is equipped with a fully charge fire extinguisher. Females' restrooms and males’ restrooms are separated from each other. All restrooms were in safe and sanitary operating condition. LPA and Site Director also toured the outdoor play area. LPA observed outdoor equipment is in good condition. The play structure appears to be age appropriate for children in care. Areas under slides and other climbing equipment is adequately cushioned. Children are signed in by facility staff when they arrived from the day school. First Aid Kit are available and Emergency supplies are properly maintained. Cleaning products and Toxins are maintained inaccessible to children.
During today's visit, a random sample of children's files were reviewed and found to be complete. LPA also reviewed staff files and staff files are found to be complete. At least one staff have current Pediatric First Aid and CPR. Facility has posted all the required licensing forms in a prominent accessible location.
