On 10/10/2024, at approximately 8:40AM, Licensing Program Analyst (LPA) Jonathan Tse conducted an unannounced annual visit at the facility. LPA met with Lead Teacher Elizabeth Lyons (S1) and explained the purpose of the visit. Present during the visit was S1, three teachers, and 25 preschool age children. The facility is in compliance with staffing and ratio requirements on this day. The facility’s operating hours are from 7:50AM to 1:50PM. Wednesdays are early release from 7:50AM to 12:50PM.

LPA and S1 inspected the facility for any health or safety hazards. LPA observed there to be a fully charged 2A10BC fire extinguisher present. There is a fire alarm system running throughout the facility. There are carbon monoxide and smoke detectors present. Emergency exits are clearly marked and visible. Evacuation routes are posted. Electrical outlets are covered or blocked when not in use to be inaccessible. Poisons, cleaning detergents, and other chemicals are stored inaccessible to children in care.

The facility is located within the premises of E.R. Taylor Elementary School. The facility uses Bungalows 1 and 2. LPA observed there to be age-appropriate toys and learning materials. Furniture is age-appropriate and free of rough or sharp edges. There are cubbies available for each child that are labeled with names. The facility uses cots for napping. Each cot is labeled with children’s names. Bedding is brought from home. The facility offers lunch and AM/PM snacks for children in care. Children have the option of bringing their own lunch from home, and hot lunch is offered and available. Water bottles are brought from home. The facility currently uses a water service. Water is provided from a dispenser.

LPA observed the outdoor play area to be free of debris and other loose articles. The outdoor play area is enclosed by a fence that is at least four feet high. It is connected to the yard of the elementary school, but the yard is kept inaccessible to children via the fence. Play structures and climbing equipment are cushioned with resilient padding.

LPA and S1 discussed a gap in a climbing ramp connecting it to a play structure. The gap is wide enough that children’s legs can fall in. Teachers will have to assist children by lifting them up from the gap. A work order has been placed to plug the gap to minimize the risk of children getting stuck. The play structure is rated for children from the ages of 2 to 5. See LIC9102-TA for technical assistance provided today.

There are no swimming pools or other similar bodies of water present. There are no firearms or weapons stored in the facility.

LPA reviewed four staff files, six children’s files, and facility records. There is at least one staff member present with current Pediatric First Aid/CPR training. All staff have completed Mandated Reporter Training. All staff members present are employed by the San Francisco Unified School District (SFUSD) and are exempt from fingerprint requirements. All children’s files were observed to be complete. The facility last conducted an emergency drill on 9/5/2024. Emergency drills are properly logged and documented. All required postings are posted in each classroom. The facility uses paper sign-in/out sheets. LPA verified that the number of children signed in matched the number of children present during today’s visit.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a
maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

No deficiencies were cited during today’s visit on 10/10/2024. See LIC9102-TA for technical assistance provided today regarding playground equipment. A notice of site visit was given and must remain posted for 30 days.
