LPA and S1 discussed a gap in a climbing ramp connecting it to a play structure. The gap is wide enough that children’s legs can fall in. Teachers will have to assist children by lifting them up from the gap. A work order has been placed to plug the gap to minimize the risk of children getting stuck. The play structure is rated for children from the ages of 2 to 5. See LIC9102-TA for technical assistance provided today.
There are no swimming pools or other similar bodies of water present. There are no firearms or weapons stored in the facility.
LPA reviewed four staff files, six children’s files, and facility records. There is at least one staff member present with current Pediatric First Aid/CPR training. All staff have completed Mandated Reporter Training. All staff members present are employed by the San Francisco Unified School District (SFUSD) and are exempt from fingerprint requirements. All children’s files were observed to be complete. The facility last conducted an emergency drill on 9/5/2024. Emergency drills are properly logged and documented. All required postings are posted in each classroom. The facility uses paper sign-in/out sheets. LPA verified that the number of children signed in matched the number of children present during today’s visit.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a
maximum of 30 days per person will be assessed if this regulation is violated.
