25 | On 5/9/2023., Licensing Program Analyst (LPA) Van conducted an unannounced visit with Regional Manager Veronica Figueroa. LPA discussed and reviewed a self-reported incident where the child informed the Education Specialist that their teacher hit them. Per the Education specialist, while observing the classroom, a child walked up to her crying and stated that the teacher had hit her. The incident occurred on 4/6/2023. According to the Regional Manager, the Education Specialist brought the incident to her attention. As part of the Center's internal investigation, staff members in the classroom were interviewed. According to the Regional manager, after speaking with the child's mom, it was revealed that after the sibling's birth, the child regressed into some behaviors and began acting entirely out of character. At the same time, the mom asked the Center's mental health consultant to provide a strategy to help with the child's behavior at home.
The staff members interviewed stated they did not see or hear any commotion. In addition, the child involved still attends the program and has a good relationship with all teachers.
Following the incident, the Center had a workshop/training for all staff on teaching Pyramind and Personal Rights training.
**No deficiency was issued at this moment.**
Regional Manager Veronica Figueroa read and signed this report.
During the visit, LPA obtained the Center's internal investigation of the incident. |
