25 | On 7/7/2023 at 2:00pm, Licensing Program Manager (LPM) Garfield Leung and Licensing Program Analyst (LPA) Hanson Leong met with Wu Yee Regional Managers Veronica Figueroa & Daisy Hsieh for an Informal Office Meeting.
During the meeting, LPM and LPA discussed an Unusual Incident that occurred on 6/16/2023 when a preschool aged child walked out of the facility unnoticed. A child was located by a community representative. According to both Regional Managers, the child left the facility for approximately one minute and forty seconds. According to both Regional Managers, the child's family was notified and made aware of the incident.
The facility quickly responded by acting, and subsequently presented and submitted its Plans of Correction to the Child Care Licensing Department. The corrective plan is as follows:
- An all staff meeting on 6/23/2023 regarding active supervision, accountability and training policies was held by the Regional Manager & Health Manager. Signatures of the staff who attended the meeting was provided during the case management visit on 6/27/2023.
- Facility will provide a monthly training about supervision.
During this meeting, LPM Leung also acknowledged the significance of the facility's actions in reporting unusual incidents to Licensing and parents. Additionally, LPM Leung discussed about the citation and civil penalty issued for this incident. In addition, Regional Managers were informed that Licensing will closely monitor compliance with the safety protocols discussed during this meeting to ensure the safety of children in care.
***The citation issued on 06/27/2023 during the Case Management Visit was also cleared on today’s visit.***
