 On August 9, 2023., Licensing Program Analysts (LPAs) Van and Leong met with the site Director, Veronica Figueroa. Today’s purpose of the inspection was to conduct a complaint investigation that the Department received on 8/1/2023. There were no children during today's visit. Director stated that the last day of school was August 4, 2023. The Center will resume the new school year on August 21, 2023.
As part of the investigation, facility physical files were reviewed, including all recent unusual incident reports the Center reported. The Center self-reported to the Department on June 20, 2023, an incident where a child opened the door in the hallway, exited through the two front doors, and left the facility. The child was found by a community ambassador and brought back to the Center. The Department investigated the incident and completed it. As a result, the Center was cited, and a civil penalty was assessed on a case management report on 6/27/2023. In addition, an Informal meeting was held on 7/7/2023 with the Center’s Administrators.
Based on interviews and record review, which were conducted, the preponderance of evidence standard has been met. Therefore, the above allegation is found to be SUBSTANTIATED.
The exit interview was conducted, and the report was reviewed with the Director, Veronica Figueroa.
