On 5/31/2024, Licensing Program Analyst (LPA) Janet Gil conducted an unannounced case management visit to the SFUSD Theresa Mahler Preschool Child Care Center. The purpose of the visit was to follow up with a self reported incident involving a child and a staff member. LPA met with lead teacher, Berta Chavez, and explained the purpose of the visit. This facility is a title 5 program operating in the SFUSD Theresa Mahler Early Education School site. All staff at the facility have caregiver background checks through the San Francisco Unified School District.

Present during inspection included 28 enrolled children, one office clerk, one lead teacher, two additional teachers, and three paraprofessional assistants. Preschool program currently operates Monday through Friday 7:30 AM to 5:30 PM. Facility is operating within its capacity and in accordance with the required ratio of staff to children.

The lead teacher stated she was involved in this incident allegation that occurred on 5/17/2024. Per lead teacher Berta Chavez, she was part of an office meeting on 5/28/2024 with the facilities principle, Carolyne Cook, along with parents and Family Support School District staff. LPA requested lead teacher to notify the department once their HR department completes its investigation.

No deficiencies were issued today during LPA's visit.

A copy of today’s report was given to the lead teacher, Berta Chavez.

A Notice of Site Visit was given and must remain posted for 30 days.
