13 | On November 21st, 2024, at approximatly 10:15 AM, Licensing Program Analyst (LPA) Janet Gil met with the substitute Priciple, David Hollands. Today’s purpose of the inspection was to close the complaint that the Department received on November 18th, 2024. Present during inspection included 41 enrolled children and 11 staff (including director and office staff). LPA inspected the classrooms indoors and the outdoor area.
As part of the investigation, facility physical files were reviewed, including recent unusual incident report the Center reported. The facility self reported an incident that occurred on October 8th, 2024 in classroom #1. The incident involved lack of supervision that resulted in a child being left unsupervised inside of the classroom. Per interview and record review of incident received, a child was left alone inside a classroom for an extended period of time. Per director, the staff failed to the do headcount resulting in a child being left alone in the classroom napping while all three staff left to supervise other children for outdoor play time for approximatly 30 minutes.
The Department investigated the incident and completed it. As a result, the Center was cited in a case management report on October 21st, 2024.
Based on interviews and records reviewed, the allegation related to the incident the Department had investigated during the case management on October 21st. Therefore, the preponderance of evidence standard has been met, and the above allegation is found to be SUBSTANTIATED.
