On April 25, 2024 at approximately 8:35am, Licensing Program Analyst (LPA) Catrina Quimbo conducted an unannounced, annual inspection. LPA met with assistant director, Mariah Lunardi, and explained the purpose of the visit. Director was not present during visit. Per assistant director, director was not present due to a pre-scheduled meeting.

Present during LPA's visit included assistant director, 3 teaching staff and 16 preschool age children. Program is operating within capacity limits and ratio during LPA's visit. All teaching staff present have fingerprint clearance on file.

Hours of operation are Monday through Friday 8:00am to 5:00pm. With assistant director, LPA inspected preschool program's day care areas, indoors and outdoors, for health and safety hazards.

Preschool program classroom includes a variety of toys and materials that were clean and in good condition. Children's furniture and equipment were observed to be age appropriate. There are multiple activity areas in the classroom that children have access to. Activity areas include writing, math, reading, art, science, dramatic play and quiet/relaxation. Per assistant director, children's toys and materials are cleaned and sanitized at least once a week or as needed.

There are carpets in the classroom that were observed to be clean, free of stains. LPA observed poles in the classroom that are covered with resilient materials. LPA observed electrical outlets in classroom to be made inaccessible with child safety covers. LPA did not observe accessible poisons, cleaning solutions or chemicals during visit.

There is sufficient storage for children's belongings located inside the classroom. LPA observed children's cubbies to be labeled with children's individual names. Facility is equipped with multiple fully charged fire extinguishers, multiple first aid kits and a fire alarm system. LPA observed multiple dual smoke/carbon monoxide detectors. LPA tested carbon monoxide detector in classroom, which was observed to be working.

There is a bathroom in the classroom that was observed to be clean and in working condition. Bathroom is equipped with appropriate toileting equipment. Additional clothes for children, if needed, were observed to be stored appropriately, labeled with children's individual names. Sinks accessible to children do not provide hot water. Garbage bins in bathroom and classroom were observed to have tight fitting lids. There is an additional reading area, located next to children's restroom that was also observed to be clean. LPA did not observe any hazardous materials in children's reading area.

Children sleep on cots. Sheets are provided by children's families and are sent home weekly or as needed. Additional sheets are available on site, if needed. LPA observed cots to be stacked and stored appropriately, labeled with children's individual names. Sheets are stored in children's individual bags to prevent sheets from touching one another. Per assistant director, children who do not sleep are given the opportunity to rest and are provided a quiet activity.

Preschool program has exclusive use of a side yard and shared use of elementary school's upper yard and gymnasium on a rotation schedule. Per assistant director, preschool children do not use elementary school's upper yard or gymnasium while elementary school children use areas.

LPA observed preschool program's side yard to be equipped with outdoor toys and materials in good condition. Per assistant director, outdoor toys and equipment are sanitized at least once a week. Flooring in side yard includes resilient padding that is vacuumed daily. There is a climbing wall in the side yard that includes resilient padding on the wall and floor. There is sufficient shade in side yard. LPA observed side yard to be fully fenced and enclosed.

Preschool children also eat in the side yard. Preschool program does not currently provide a food service. Children's families provide children with their lunches. LPA observed children's individual lunch boxes to be labeled with children's individual names.

Elementary school's upper yard and gymnasium were also inspected during visit. Preschool children use areas by shared play time and individual play time. Preschool program's exclusive toys and equipment are stored in areas. Per assistant director, children are supervised going to and from areas.

Water is made available to children, both indoors and outdoors, through children's individual water bottles and water pitchers with cups that are refilled on site through filtered water stations.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care
Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

During today's visit, LPA reviewed seven random children's files. Children's files were observed to have a record of immunizations, emergency and admission information. LPA also reviewed teaching staffs' files. Teaching staff present have proof of required immunizations and educational transcripts, that were made available for review. During LPA's review of files, LPA observed no preschool teaching staff present have a current CPR/First Aid certification. Director has a current CPR/First Aid certification, however, is not present during LPA's visit.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871,
must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00
minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a
maximum of 30 days per person will be assessed if this regulation is violated.

Facility representative was informed of the MyChildCarePlan.org website; a
consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During today's visit, no preschool teaching staff have a current CPR/First Aid certification. Preschool program was issued a Type B citation. A plan of correction was discussed with assistant director. Appeal rights were provided during visit.

A notice of site visit was given and must remain posted for 30 days.
