On September 22, 2023., Licensing Program Analyst (LPA) Van met with Director Elizabeth Perla for an unannounced required annual inspection. The purpose of the inspection was explained. Four preschool children were supervised by 2 teachers today. The facility complies with the teacher-children’s ratios. This is a combination Center with Infants, Preschool, and School-age programs operating in the same building. All staff members have criminal record clearances. Hours of operation are Monday- Friday, 8:30 am- 5:30 pm.

During the inspection, LPA thoroughly inspected the physical plant to identify any potential health and safety hazards. The facility maintains an accurate sign-in/out record for children, and all parents' signatures were accounted for and matched the number of children present. The facility is well-maintained, with proper temperature and ventilation. The preschool classroom is equipped with age-appropriate toys and learning materials, and the furniture is sturdy and in excellent condition. Additionally, the classroom has a carbon monoxide detector, centralized smoke alarm, fully charged fire extinguisher, and working telephones. LPA noted that each child has their own cubby for personal belongings. All emergency supplies are kept in locked storage, and first aid supplies are fully stocked and accessible to the children. Furthermore, all cleaning solutions, poisons, and other hazardous chemicals are appropriately stored in a high cabinet in each classroom, ensuring they are inaccessible to the children.

All toilets and handwashing sinks are in good working condition with proper sanitation. LPA observed no bodies of water on the premises. The facility has an outdoor waiver approved by the Department that allows no more than 13 children combined in three programs at one time. LPA reviewed the waiver and reminded the Director that infants, preschoolers, and school-age children may not be commingled with any other age group while in the outdoor play area. Additionally, the group should not exceed a capacity of 13 at any time. LPA observed that the outdoor playground area is in good repair and completely fenced for supervision. Outdoor toys are age-appropriate, in good condition, and free of loose parts. Climbing and slide equipment have soft cushions underneath to break any accidental fall.

Both children's and personnel records were reviewed. These records showed that the facility has detailed information about each child's authorized representative, including their names, addresses, and telephone numbers. Additionally, the files contain records of each child's immunization, medical assessment, and admission agreement. The staff records indicate that all individuals who require caregiver background checks have undergone criminal records and child abuse index clearances or exemptions. All staff members have valid CPR certifications and maintain up-to-date immunization records. The educational qualifications of the teachers were also reviewed, and it was found that all teachers had completed the required ECE units.

The facility had posted the required forms. (i.e., License, Notification of Parent's Rights, Notification of Personal Rights, Car Seat Law, Emergency Disaster Plan, and Daily activities). The daily schedule is observed and posted. The Center also has water pitchers available for children indoors and outdoors. All containers for solid waste within the Center have a proper tight-fitting cover.

LPA reminded the Director that as of September 1, 2016. A person may not be employed or volunteer at a childcare facility unless he or she has been immunized against influenza, pertussis, and measles.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies and reviewed children’s, personnel, and administrative records.

The Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

THE CENTER COMPLETED TESTING AND NO LEAD EXCEEDANCES:
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5 years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

The Director was informed about Mandated Reporter training available on the CCLD website. In addition to AB1207 training required to be completed every two years by all staff hired a one-time General Training is also required. Training can be taken online at www.mandaterreporterca.com. Also, the Director was reminded of Pesticide training. Information on the DPR website at: www.cdpr.ca.gov/shoolipm/childcare

The Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

To improve the quality and value of the new inspection process, a survey will be sent to the email address provided. Please complete the survey and share your inspection experience. During the inspection, LPA requested a new Emergency Disaster Plan (LIC 610), Designation of Responsibility (LIC 308), Personnel Report (LIC 500), and Incidental Medical Services (IMS) Plan. All requested documents will be submitted to Licensing by October 6, 2023.

A notice of site visit was given and must remain posted for 30 days.

Due to the Director's having to leave during the inspection. The Director has assigned her designee, lead teacher, Jessica Duong to review and sign the report. Exit interview conducted and report was reviewed with the lead teacher, Jessica Duong.
