On 9/8/2022 at 8:50AM., Licensing Program Analyst (LPA) Luis J. Gomez met with Principal, Valerie Hoshino. Purpose of the inspection was explained and was for unannounced complaint investigation. Present was the Principal and 11 staff supervising 37 children. LPA inspected facility, inside and outside, for health and safety hazards.

During inspection, LPA conducted site observations, interviewed staff and reviewed the facility records. At 9:15AM., Based on observations, LPA confirmed broken glass tank loctaed in the outdoor play area. During inspection Advisory Note: Technical Violation (LIC9102TV) was issued. Broken tank was removed during inspection.

During the course of the investigation, site observations were conducted on 7/7/2022 and 9/8/2022. A review of the facility records was also complete, which included the children/ staff files and attendance records. LPA conducted interviews with the Principal, Staff, and all involved parties. Regarding the allegation of facility is out of ratio; Based on interviews, record review and evidence collected, LPA was unable to determine if facility is out of ratio. During inspection, LPA observed preschool classrooms operating within the required Teacher- Child ratio.

Regarding allegation of unqualified staff providing care to day care children; Based on record review and interviews, LPA was unable to determine if staff is unqualified. During record review, LPA confirmed classroom teachers are fully qualified.

Although the allegation(s) may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations(s) did or did not occur, therefore the allegation is Unsubstantiated.

LPA conducted exit interview with the Principal, Valerie Hoshino. Report was explained and Notice of Site Visit was posted during inspection.
