At 11:45AM LPA review facility records including five children’s files and five personnel files. Staff files reviewed and included: Proof of Education Requirement, Notice of Employee Rights (LIC9052), Criminal Record Statement (LIC508) and Declaration to Report Suspected Child Abuse (LIC9108). Per principal, yearly mandated reporter certification training is conducted yearly through SFUSD.
Children’s files reviewed and included the: Consent for Medical Treatment (LIC627), Proof of Updated Immunization Record, Identification of Emergency Information (LIC700), Personal Rights (LIC613A) and Notification of Parent’s Rights (LIC995).
Present staff members had their current CPR/ 1st aid certification, which expires on: 8/20/2023. Emergency disaster drills are conducted every six months with the last drill done on, 10/20/2022 and properly logged. Facility provides all daily meals. Refrigerators inspected are clean and in proper use. Food items serviced were current.
LPA observed required posting in facility, including: License, Parent’s Rights (PUB393), Emergency Disaster Plan (LIC610) and Outdoor Space Waiver. LPA reminded facility to posted updated the lunch menu. Advisory Note: Technical Assistance (LIC9102) was issued.
LPA reminded facility to update Emergency Disaster Plan (LIC610). During inspection, LPA reviewed children’s medication stored in the classrooms. LPA reminded facility to ensure all medication is stored in original container. Advisory Note: Technical Assistance (LIC9102TA) was issued.
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manuel – Regulations Interpretations and Procedures for Child Care Centers Section 101173 and 101226. When an IMS is provided, an updated Plan of Operations that includes IMS must be submitted to the Department.
