At 10:35AM., LPA observed the following: Classrooms were clean with a variety of age-appropriate books, blocks and supplies for the children. All floors and surfaces leading to exits were free of obstructions. Classrooms had labeled cubbies for storage of child’s belongings. Blocks, books and supplies were free of sharp edges or splinters. Classrooms had child sized tables and chairs for snack and seated activities. Fixtures in children's bathrooms were in operating condition. Bathrooms had adequate supplies for hand washing. Staff bathroom is located separately. For napping services, program provides napping cots, assigned to each child. Per staff, napping supplies are taken home weekly to be washed. Classrooms had acceptable ventilation and lighting. Detergents, cleaning supplies and toxins are stored inaccessible to children. Classrooms have functioning smoke detector, carbon monoxide detector and fire extinguishers (2A:60BC) located in the hallway. First aid kit was fully stocked.

At 11:10AM., LPA inspected the outdoor play area. Outdoor play area was enclosed with tall fencing and equipped with shading and rest area. Space was observed free of debris or hazardous plants or objects. Playthings inspected were in good repair and play structure had been properly anchored. Water services are provided for day-care children using accessible fountains. (REFER TO 809-C FOR CONT.)

At 11:45AM LPA review facility records including five children’s files and five personnel files. Staff files reviewed and included: Proof of Education Requirement, Notice of Employee Rights (LIC9052), Criminal Record Statement (LIC508) and Declaration to Report Suspected Child Abuse (LIC9108). Per principal, yearly mandated reporter certification training is conducted yearly through SFUSD.

Children’s files reviewed and included the: Consent for Medical Treatment (LIC627), Proof of Updated Immunization Record, Identification of Emergency Information (LIC700), Personal Rights (LIC613A) and Notification of Parent’s Rights (LIC995).

Present staff members had their current CPR/ 1^st aid certification, which expires on: 8/20/2023. Emergency disaster drills are conducted every six months with the last drill done on, 10/20/2022 and properly logged. Facility provides all daily meals. Refrigerators inspected are clean and in proper use. Food items serviced were current.

LPA observed required posting in facility, including: License, Parent’s Rights (PUB393), Emergency Disaster Plan (LIC610) and Outdoor Space Waiver. LPA reminded facility to posted updated the lunch menu. Advisory Note: Technical Assistance (LIC9102) was issued.

LPA reminded facility to update Emergency Disaster Plan (LIC610). During inspection, LPA reviewed children’s medication stored in the classrooms. LPA reminded facility to ensure all medication is stored in original container. Advisory Note: Technical Assistance (LIC9102TA) was issued.
