25 | Licensing Program Analyst (LPA) Mok, conducted an unannounced case management for an unusual Incident reported by the licensee on 6/11/2024. LPA met with the Site Director, Yanina Escalante, and explained the purpose of the inspection to her. There were 14 children with 3 staff present. Regarding the unusual incident on 6/2/2024, a child opened a classroom door and walked out of the classroom to the hallway by himself for about 30 seconds; a child was discovered by the Resource Center Manager when she was in the hallway during her visit. The manager supervised and walked the child back to the classroom immediately. The licensee had a plan of correction to prevent a similar incident by installing a childproof knob to make the door handle move upward to open and a door chime to the door in each classroom, and providing Care and supervision review training to all staff on 6/14/2024.
