25 | Licensing Program Analyst (LPA) Mok conducted an unannounced case management inspection today. LPA met with the Site Director, Nkechi Nwankwo. The purpose of the inspection was explained to her. There were 29 children with 5 staff present. The facility submitted an unusual incident report to CCL on 3/14/2022. The incident related to a child who fell and hit his/her head on the edge and resulting in a cut on the left side of the child's forehead. The child was taken to the hospital and got a couple of stitches. The child came back to school the next day. The child is fine now. The incident was an accident. The child is in the special Ed's program. According to the documents, the child has the Occupational Therapist and Physical Therapist work on the child's Motor Development. LPA obtained the relevant documents from the Site Director during the inspection.
