On 4/27/2023 at 12:40PM., Licensing Program Analyst (LPA), Luis J. Gomez met with Teacher, Inna Plamadeala. Purpose of inspection was explained and was for an Unannounced, Annual Random inspection. Facility is licensed to operate preschool-age program at San Francisco City College: Mission Campus. Director, Melissa Serrano arrived during inspection. Present was the directors and 3 staff supervising 8 children. Children present had been properly signed in. Staff have criminal record clearances on file. Preschool program utilizes three classrooms: Red Room; Rainbow Room; Blue Room/ Gross Motor; and shared, Napping Room; Dinning Area (Kitchen) and Outdoor play yard. Program operates school year program (9 -month program). LPA inspected facility, indoors and outdoors, with director for health and safety hazards.

At 12:45PM., LPA observed the following: Classrooms were maintained clean, orderly, with plenty of age-appropriate playthings available for the children. Floor and ground surfaces were free of any obstructions or potential hazards. Labeled cubbies are available in hallway for children's belongings. Children's furniture, art supplies, and books inspected were like-new condition. Puzzles were free of sharp corners or splinters. Facility was equipped with child- sized tables and chairs. Children’s bathrooms, located in each classroom, were clean and fixtures in operating condition. Staff bathroom is located separate. For napping services, stackable cots are stored in the shared napping room. Cots inspected were made of cleanable/plastic material. Per director, blankets and sheets are not shared between children and are washed weekly. Classrooms had acceptable ventilation and lighting. The disinfectants and poisons have been stored inaccessible to children. Accessible outlets and trash bins have coverings installed. Facility has built-in smoke detector, functioning carbon monoxide detector, telephone service, and fully charged fire extinguishers: 2A:10BC. Stocked first aid kit is located in the red classroom.

At 1:15PM, LPA inspected the shared outdoor play area. Outdoor play yard is enclosed, with playthings inspected in good repair. Rubber padding had been installed for added safety. For water services, director stated refillable water bottles are provided for children with pitchers. Accessible shading rest area with bench is available. Shed was reviewed during inspection.



LPA inspected kitchen/ food storage area. Food containers inspected were current and stored correctly. Storage cabinets and refrigerators were clean, without trash or rubbish.

At 1:45PM LPA reviewed facility records including 6 children’s files and 4 personnel files. Staff’s files contained the: Notice of Employee Rights (LIC9052); Education Verification; updated 'Mandated Reporter Training' certification (AB1207); and Personnel Record (LIC501).

At 2:35PM., Based on interview and observations, LPA confirmed staff, S3, present without proof of required immunization on file. Advisory Note: Technical Violation (LIC9102) was issued.

Children’s files were reviewed and contained the: Consent for Emergency Medical Treatment; Immunization Records, Identification and Emergency Information; Personal Rights (LIC613A); and Notification of Parent’s Rights (LIC995).

Staff member, S2, had the ‘Cardiopulmonary Resuscitation (CPR) and First Aid’ certification, expiring on: 11/2023. Emergency disaster drill are conducted every six months, with the last drill completed on 1/12/2023, properly logged.

LPA observed required posting in facility, including the: Facility License; Notification of Parent’s Rights (PUB394); Emergency Disaster Plan (LIC610); and Seat Belt Safety Laws.

LPA reminded director to ensure snack menus is posted, at least one week in advance. Advisory Note: Technical Violation (LIC9102TV) was issued.

Per director, facility provides afternoon snack and parents provide lunch.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manuel – Regulations Interpretations and Procedures for Child Care Centers Section 101173 and 101226. When an IMS is provided, an updated Plan of Operations that includes IMS must be submitted to the Department. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in Child Care Center. A civil penalty of $100.00 minimum/ day up to $500.00 maximum per day/per person will be assessment if this regulation is violated.

Based on today's inspection, no deficiencies were observed in the areas evaluated according to the Title 22 Division 12 Chap. 1 Ca. Code of Regulations. Exit interview and Report was reviewed with Director, Melissa Serrano, and signature of this form acknowledges receipt of these documents.

LPA unable to print report during inspection. Copy of report and notice of site visit will be mailed to director at a later date.
