On February 28,2020, at 1:00 pm, Licensing Program Analyst (LPA) Van met with the lead teacher, Natalia Kaplan, for an unannounced annual random inspection. The purpose of the inspection was explained and was granted entry to the facility by the staff. The facility is a preschool program and operating in the Congregation B’nai Emunah Church. Present today are 2 teachers, and 2 helpers supervising 18 children; The Director is off today. Staff to children ratio are met. Days and hours of operations are from Monday - Friday 8:00 AM - 5:50 PM. There are five classrooms; a common area room, the PE room, the playroom, and the activities room, and Napping room. The facility has three bathrooms with a total of 3 toilets and 3 sinks, and all toilets and sinks are clean and well sanitized.

LPA along with the lead teacher inspected the daycare areas for health and safety hazards. The following were observed during the inspection. The facility has a sign-in / out record with full legal signatures from the parent or authorized representative. There are variety of age-appropriate toys and learning materials for children are available; the center is in good repair, free of any hazards, and has proper temperature and ventilation. The classroom is set up for childcare; furniture, equipment, sufficient amount of sleeping mats for children are available. All children's chairs and tables have rounded corners. LPA observed no pools, spas, or other bodies of water at the facility. The facility has a smoke detector and carbon monoxide detector fully charged fire extinguisher and a working telephone at the site. All cleaning solutions, poisons, and other chemicals that are dangerous stored inaccessible to children. The outdoor playground area is in good repair. Play structures are free of any loose parts. The facility has posted the required forms (i.e., License, menus, waivers, Notification of Parent's Rights, Notification of Personal Rights, Car Seat Law, and Emergency Disaster Plan).

Per the lead teacher, there are no firearms, weapons, or pets at the center; the center provides two snacks, breakfast and lunch for children. The food preparation area is clean, organize, and foods store appropriately. Water bottles are provided to the children by the center and water filter pitchers are used to refill the bottles. The lead teacher stated that Fire and Earthquake drills are done quarterly. The last fire drill was completed on 12/04/19.

At 2:00 pm, LPA reviews the children’s and staff’s file, Care and Supervision of the Children, and Medication Policies. A review of staff records during today’s visit indicates that all staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. All staff have a record of valid Pediatric FirstAid/CPR card in file. Also, all staff have the required immunization and verification of the mandated child abuse reporting training as compliant with AB1207. At 2:30 pm, during the children's records review, LPA found that C3 and C6 Parents' rights LIC 995 form were incomplete or missing parent's signature.

LPA reminded the lead teacher that as of September 1, 2016. A person may not be employed or volunteer at a childcare facility unless he or she has been immunized against influenza, pertussis, and measles.

LPA reviewed AB 1207 with the lead teacher. As of January 1, 2018, all staff is required to complete Mandated Reporter Training every two years. The training can be obtained online at www.mandatedreporterca.com.

ADA information was provided; US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to the publication. Pesticides training was also discussed (Information on the DPR website at www.cdpr.ca.gov/shoolipm/childcare). Incidental Medical Services (IMS) Plan was also discussed with the lead teacher.

During the inspection, LPA requested a new Emergency Disaster plan, Designation of Responsibility, and Personnel Report. The lead teacher states she will send to licensing. LPA also went over the Child Care Center checklist and provided a copy to the lead teacher during the inspection for reference in the future inspection.

See 809D for deficiency that was cited today. The copy of this report is reviewed and provided to the director. Notice of site visit is posted and shall remain posted for the next 30 days.
