25 | On June 14, 2024, Licensing Program Analyst (LPA) Van conducted an unannounced case management meeting to discuss the incident reporting requirement. The purpose of this meeting was explained to the Assistant Director. During the inspection, the assistant Director briefly escorted her two school-age children to their school and returned promptly. Twelve regular teachers and two floaters supervised 60 preschool children, meeting the required teacher-to-child ratio. The Assistant Director mentioned that all teachers must have a minimum of 12 ECE units. Site manager Wendy Tong arrived during the visit.
The center submitted two incident reports, but one was sent two weeks after the incident occurred. The second report did not meet the proper reporting requirements. During the inspection, the LPA discussed the reporting requirements with the assistant director, emphasizing her crucial role in ensuring timely and accurate reporting. It was reiterated that a report must be made to the Department by telephone or fax during regular business hours on the next working day. Additionally, a written report with the necessary information must be submitted to the Department within seven days of the incident.

A notice of site visit was given to the assistant Director and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
An exit interview was conducted, and the report was reviewed with the assistant Director, Helen Lao Tong. |
