13 | Licensing Program Analysts (LPAs) Mok and Singh conducted an unannounced inspection today. LPAs met with the Site Director, Juan Pazimo. The purpose of the inspection was explained to him. There were no children present. LPAs interviewed staff during the inspection. According to the information from the interviews, children walked out the classrooms without teacher permission to meet up their parents at the parking.
Based on LPAs interviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12 & Chapter 3), are being cited on the attached LIC 9099D.
An exit inspection was conducted with Site Director and appeal rights were explained. A printed copy of the report, as well as a printed copy of the appeal rights, were provided to the Licensee at the conclusion of the inspection. Notice of site visit was posted and must remain posted for 30 days for public review. A Type “A” violation (see continuation) was issued today. The center is informed to provide a copy of the Evaluation Report and the Type “A” Deficiency cited to the parents and guardians of children currently enrolled in care and to parents of newly enrolled children during the next 12 months. A signed and dated LIC 9224 shall be maintained in all Children's files. This report and appeal rights were provided and reviewed with the TVS. Notice of Site Visit shall remain posted for 30 days.
