13 | Licensing Program Analyst (LPA) Rodriguez arrived at the facility unannounced to close the investigation into the above allegations and met with Kim Wong and Juan Pazmino. During the investigation, LPA interviewed staff and witnesses regarding the above allegations. Per LPA interviews, an incident did occur on 11/21/2019 at the facility involving a few students in the after school program. This incident was reported the facility on 11/25/2019, who subsequently cross reported the incident to licensing department on the same day. Parents of the students were notified of the incident at the next available date. In addition, LPA Rodriguez's interviews with staff did not produce a disclosure that there was a lack of supervision or a disclosure of retaliation towards a day care family by FacesSF.
There have been conflicting statements made from all parties about the above allegations; such LPA is unable to determine whether or not the allegations actually occurred.
Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore, the allegations are closed as unsubstantiated. A printed copy of the report as well as the notice of site visit was provided to Kim Wong and Juan Pazmino. |
