LPA observed that six children were not signed into the facility. LPA observed that the facility did not document the infants sleeping behavior every fifteen minutes. LPA observed that the facility did not have the parents complete the LIC 9227, Individual Infant Sleeping Plan, for each infant. LPA informed the director that the facility was issued three technical violations as a reminder to comply. Children are required to bring their own food to the facility,
LPA reviewed the records of seven staff members, A review of the facility's records revealed that the names, addresses, and phone numbers of each child's authorized representative kept on file. A review of staff records revealed that two of the staff members did not have current CPR and First Aid certifications. A review of staff records also revealed that one staff member did not complete their mandated reporter training, LPA informed the director that the facility was issued a technical violation as a reminder to comply.
According to the Director, the Center simulates and documents fire and earthquake drills every three months.
Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA also informed the Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased
