On 6/24/2022 at 8:35AM., Licensing Program Analyst (LPA), Luis J. Gomez met with Lead Teacher, Steven Corona. Purpose of the inspection was explained and was for an unannounced; annual random inspection. Present was three staff supervising 12 children. All children were properly signed in. Staff had their criminal record clearances on file. Preschool program utilizes one classroom and one outdoor play space: Hours of operation are Monday- Friday 8:00am- 4:30pm. Program operates year-around. LPA inspected facility, indoors and outdoors, for health and safety hazards.

At 8:50AM., the following was observed: classroom was clean, orderly, and with age-appropriate playthings available for the children. Floor and ground surfaces were free of obstructions. Children’s furniture and items inspected were in proper repair. Facility lobby has individual cubbies for storage of children's belongings. Classroom had child size tables and several chairs. For napping services, labeled cots were stored closet with each child’s name. Per staff, napping sheets are washed weekly. Children’s bathrooms were reviewed during inspection. Bathroom fixtures were in operating condition with adequate supplies available for the children.

LPA advised lead teacher to replace safety lock on bathroom storage cabinet. Staff bathroom is located separate. Classroom was the proper temperature, with ventilation, and lighting. Detergents, cleaning supplies, and all other toxins were made inaccessible. Outlets had been properly covered. Facility had functioning telephone service, smoke/ carbon monoxide detector combo, and fully charged fire extinguisher; 2A:10BC. First aid kit was review during inspection. At 9:05AM., LPA inspected the Outdoor Play Area. Outdoor spaces was enclosed, with adequate shading, and play structures for the children. Play structure was properly anchored. Soft rubber padding was installed to absorb potential falls. Water pitcher with cups are accessible for the day-care children. Per lead teacher, pitcher is refilled daily from non-contained source.

At 9:10AM., Based on observations, LPA confirmed tricycle in outdoor play area missing the right handle bar covering. Advisory: Note Technical Assistance (LIC9102TA) was issued. Damaged item was moved to the off-limit area during inspection.

At 9:25AM. LPA inspected the facility kitchen. Program provides daily lunch and snack services for children in care. All food items reviewed were current and properly labeled.



At 9:40AM., LPA review the facility records including ten children's files and five personnel files. Facility files were reviewed and included the: Criminal Record Statements (LIC508); Declaration to Report Suspected Child Abuse (LIC9108); Transcripts; Proof of Immunization; and Notice of Employee Rights (LIC9052). Mandated reporter training certifications on file were current.

Children's files reviewed were complete and included: Health History (LIC702); Identification of Emergency Information (LIC700); Consent for Medical Treatment (LIC627); Notification of Parent’s Rights (LIC995); and Immunization Records.

Staff member (S3) CPR; (Cardiopulmonary Resuscitation)/ First Aid certification was current; expiring 4/2023.


LPA reminded lead teacher to conduct and log required emergency disaster drills every six months.

Required posting were posted and include: Childcare License, Notification of Parents Rights (PUB393); Updated Emergency Disaster Plan (LIC610); Snack Menu; and Child Passenger Safety Laws (REFER TO 809C, FOR CONT.)

Lead Teacher was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in Child Care Center. A civil penalty of $100.00 minimum/ day up to $500.00 maximum per day/per person will be assessment if this regulation is violated.

Based on today's inspection, no deficiencies were observed in the areas evaluated, according the Title 22 Division 12 Ca. Code of Regulations. Exit interview was discussed with Lead Teacher, Steven Corona and signature of this form acknowledges receipt of these documents.



The 'Notice of Site Visit' was provided to director and must remain posted for 30 days.
