On November 15, 2024, at approximately 11:00pm, Licensing Program Analyst (LPA) Ly, arrived at this Child Care Center unannounced to close the complaint investigation to the above allegations and met with the Director Rebecca Cullinan. Purpose of visit was explained to the Director. Present during the visit were the Director and 4 staff caring for 5 toddler age children and 13 preschool age children.

Based on information gathered through interviews and records reviews, the agency has investigated the complaint allegations above, there was no sufficient evidence to allegations Staff are operating beyond the terms and conditions of the license, Staff do not provide adequate care and supervision, Staff prevents the daycare children from using the restroom. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur; therefore the allegations are UNSUBSTANTIATED.

This report was reviewed with the Director whose signature confirm have read the report. Report must be made available for public review upon request. A copy of this report and rights to comment and appeal have been discussed and left with the Director. Notice of Site Visit was provided and Notice shall be posted for 30 days.
