Licensing Program Analysts (LPAs) Glenn Schnell and Marie Rodriguez met with Applicant Jiemin “Tiffany” Zhang today for a Pre-Licensing Inspection. This inspection included a technical assistance inspection for COVID-19 guidance and support. The applicant is requesting 55 children (ages 2 years to entry into first grade). Applicant is also requesting a toddler option component as well for 12 children in the total capacity of 55 children. The facility will operate from Monday - Friday; 7:30 AM to 6:00 PM. The designated director/site supervisor for the facility is Chun Yan “Sadira” Wang. The facility was inspected today, indoor and outdoor, for health and safety hazards and measured to calculate capacity.

Indoor: There are three classrooms that will be used for the preschool program, one classroom for the toddler program, and a side quiet room to be used by both programs. Total useable indoor area measures at 2,031.43 sq ft divided by 35 sq ft equals 58 children. Applicant is requesting a total capacity of 55 children. The indoor space for children is observed today to be clean and all furnishings, toys, and equipment observed to be safe and in good condition. There is a storage space for children's personal belongings and mats will be utilized for napping children. Parents will provide a sheet and blanket for their child and bedding will be sent home to be laundered weekly. Mats will be cleaned at least weekly or more often if needed. Children will be signed in and out manually, but the applicant has plans for implementing an electronic sign in and out system. The indoor space is equipped with a fire extinguisher, smoke and carbon monoxide detectors, a separate kitchen area where snack food is stored, garbage cans with tight-fitting lids, a first aid kit, and emergency supplies. The facility is adequately ventilated and free of insects and bugs. During the COVID-19 pandemic, the program will not provide food services. Children will bring their own foods to eat each day. In the future, the program will provide two snacks daily. Long term plan will include parents paying an extra fee for catered lunches, as an optional service. Only disposable products will be used for food service since the facility is not set up to provide adequate sanitization of dishes and utensils.


There are four toilets and seven sinks accessible for children's use (three sinks in bathroom, two sinks in toddler classroom, and two sinks in preschool classroom). Bathroom is located between the preschool and toddler classrooms. The bathroom will be shared between programs and a schedule will be created for use. Diaper changing will be done in the bathroom with a portable changing pad. The bathroom will be cleaned and disinfected in-between scheduled use of each classroom and between use by each child. The bathroom was observed to be clean and with no health and safety hazards. Sinks accessible to children will only deliver cold water. A separate bathroom is available for staff.

Snack foods will be stored in the kitchen separately from any cleaning products or other hazards. All toxins and other hazardous items will be stored in the cabinet above the refrigerator and will be inaccessible to children. Medications will be stored in the front office area inaccessible to the children. Refrigerated medication will be locked in a secure bag for each child and placed in a drawer designated only for medication. During the COVID-19 pandemic, children will bring in their own reusable water bottle to use and water refills will be dispensed by staff. Long term plan of the programs will be to provide a pitcher with water and disposable cups for children to drink water indoors. Isolation of ill children will be in the small activities room next to the office and staff bathroom will be used for an ill child. Continued on next page.

Outdoor: The outdoor areas measure a total of 1698 sq ft. allowing for a total of 23 children. LPA advised that a waiver will need to be approved for scheduled rotational usage. LPA Schnell received a request for a waiver and rotation schedule during today's inspection. The rooftop outdoor area to be used is enclosed by at least a five foot fence since it is located on a rooftop, and is observed to be clean, safe and equipped with toys and equipment for children. Resilient material located throughout the play area is a pour and play soft padding covered by a fake turf/grass. There is a secure locked gate at the top of the stairwell to the rooftop outdoor area. Lower outdoor play area does not have a secure gate at the bottom of the stairwell leading to the rooftop area in accordance with fire safety code by San Francisco Fire Marshall. During outdoor play time in the lower outdoor area, a teacher will be stationed at the bottom of the stairwell for supervision. During the COVID-19 pandemic, for outdoor access to drinking water, children will bring in their own reusable water bottle to use and water refills will be dispensed by staff. Long term plan is there will be a pitcher and disposable cups to allow children access to drinking water outside. Shade for children will be provided by portable shade structures.
The following items were reviewed as part of today's visit: Record keeping for staff, children, and facility records, Care and Supervision of the Children, Child Discipline Procedures (staff will talk with the children and use redirection with a book in the quiet area), Emergency Evacuation Procedures, Medication Policies (non-prescription medications will not be administered), Isolation of Sick Children, Napping Requirements, Food Service, Transportation (none will be provided), Parents Rights, and Reporting Requirements. Required postings were observed during today's inspection. Facility will conduct monthly disaster drills and a log will be maintained for review upon request. The fire inspection was conducted, and approval was received on 4/21/21. The requirement for Lead Water Testing was discussed (H&S Code 1597.16). Information was provided to applicant during today's inspection. Advisory Note provided to applicant stating that the requirement for testing must be met by January 2023. Continued on next page.

This facility will provide Incidental Medical Services (IMS). LPAs reviewed storage area for medication and equipment/supplies, and reviewed forms that will be used. LPAs Schnell and Rodriguez conducted a Pre-Licensing Inspection today and the inspection included providing the applicant with the COVID-19 self-assessment guide. LPAs also reviewed COVID-19 required postings and provided additional COVID-19 resources to applicant. Applicant was informed that access to available Personal Protective Equipment (PPE) may be available through the local childcare resource and referral agency.

An electronic copy of this four page report was emailed to applicant Jiemin “Tiffany” Zhang on 5/19/21. Applicant was asked to review the report and return a signed copy of the report to LPA Schnell.

LPA Schnell will recommend provisional licensure for this facility for a capacity of 55 children once the following has been received. Provisional License period will be effective 5/19/21 through 6/18/21:

Remaining paperwork for Sadira Wang: a copy of Education transcripts, a copy of Immunization Records for MMR, tDAP, and influenza shot (or a signed written declaration declining influenza shot), LIC 503 Health Screening Report, a copy of tuberculosis test results (within one year), and copies of the following certificates: updated Pediatric CPR/First Aid Training, Lead Poison Training, Mandated Reporter Training, and Pesticides Training.
