Licensing Program Analyst (LPA) Haydee Caliboso met with the applicants Helena Geng, Co-Founder, and Jennifer Knappick, Director, for a pre-licensing inspection. The applicant has requested a capacity of 24 children ages birth to 24 months. This facility currently has an existing preschool program for children ages 2 years to entry into first grade that will operate with the infant program. The applicant has also requested to move the infant classroom to one of the preschool classrooms located in the second building of the facility facing Ringold St. The facility will operate Monday – Friday, 7:00AM – 6:00PM. The facility was inspected today, indoor and outdoor, for health and safety hazards.

Indoor:
The room to be used for the infant program is in the second building facing Ringold St., which was previously measured, 954 sq. ft, and will accommodate a capacity of 24 children (see updated facility sketch). LPA advised the applicant/director that the infants are not to be co-mingled and are required to operate separately. The infant classroom has a designated classroom in the second building. Parents will provide an individual written plan for each infant, and formula or milk for bottle-fed infants. Bottles and food shall be labeled. Food and milk that is not consumed is sent home daily. Staff will meet and review individual written plans quarterly with the parents. There are 2 changing tables in the infant room and a sink within arm’s reach of the child. Parents will provide diapers and wipes for their children. There are 2 toilets and 2 sinks available for children to use in the infant classroom.

The classroom and furniture equipment are clean and in good condition. Children’s toys and supplies are orderly, in good condition, and age appropriate. There is storage space for children's personal belongings. There is a separate napping area that is currently equipped with 3 cribs. The Co-Founder has stated, additional cribs will be purchased as enrollment increases. Parents will provide a sheet and blanket for their child. Bedding is sent home to be washed weekly or as needed. The facility is equipped with a fire extinguisher, and both smoke and carbon monoxide detectors. The facility has a kitchen with built-in counter-top area, with a working refrigerator for food preparation located in the other building. The infant classroom has a working refrigerator where bottle and food are stored. The portable bottle warmer is made out of reach of children and is located inside the Director’s office. The infant classroom has garbage cans with tight-fitting lids. The facility has a designated backpack with a first aid kit and emergency supplies. Refrigerated medications will be stored separately from food. Medication will be administered with parental consent and will be kept in the child’s file for record. All medication will be logged when administered to children. The program will provide lunch, morning, and afternoon snacks daily. Only disposable products will be used for food service. There is a designated bathroom for staff to use.

Outdoor:
The playground area that will be used in the facility is located between the two buildings, attached to Infant classroom. The square footage for outdoor was previously calculated for 685 sq. ft, allowing for a capacity of 8 children. The facility provided a request waiver for scheduled rotation for outdoor usage. No more than 8 children may be in the play area at a time. The facility’s outdoor playground is enclosed and gated by steel fence approximately 10 ft. tall. The outdoor is separated into two areas. The playground area where children will be present during outdoor time is enclosed with fence planters approximately 5 ft. tall. No children are allowed on the other side of the facility’s outdoor space. The outdoor space was observed to be clean, safe, and equipped with toys that are age-appropriate for children. There is structure steel shade for children. The playground has no climbing structures or slides. LPA reminded Helena and Jennifer to ensure ratio and supervision are observed at all times. The facility will conduct and log fire drills once every six months, and the log will be available upon request.

The following items were reviewed as part of today’s visit: Record keeping for staff, children, and facility records, care and supervision of the children, child discipline procedures – staff will use positive discipline. Emergency Evacuation Procedures, Medication Policies, Isolation of Sick Children, Napping Requirements, Safe Sleep and Individual Infant Sleeping Plan (LIC 9227), Food Service, Transportation-none provided, Parents Rights, and Reporting Requirements were reviewed today.

This facility will provide Incidental Medical Services – IMS. LPA reviewed storage area for medication and equipment/supplies, and reviewed forms that will be used. Based on available square footage, sinks, and toilets, LPA Haydee Caliboso will recommend licensure for the infant program for a capacity of 24 children. The following is required prior to licensure:
· Fire clearance approval from San Francisco Fire Department
· Decrease in capacity of the preschool license from 66 to 39 children is required.
· Updated LIC 200A for decrease in capacity for the preschool license.
· Letter of waiver for shared usage of the outdoor play area.
