Licensing Program Analyst (LPA) Glenn Schnell met with Applicant Ashley Song today for a pre-licensing inspection. This inspection included a technical assistance inspection for COVID-19 guidance and support. The applicant is requesting a combination center for 36 preschool children (ages 2 years to entry into first grade), 34 infants (ages 0-2) and a toddler option component with the infant application for 12 children (ages 18 months to 36 months). The facility will operate Monday - Friday; 7:30 AM to 5:30 PM. The designated director/site supervisor for the facility is Shuai Wang. The preschool facility was inspected today, indoor and outdoor, for health and safety hazards and measured to calculate capacity. The Preschool inspection will be documented on another Facility Evaluation Report.

Indoor Infants: There is currently one enclosed classroom for the infants, one enclosed classroom for the toddler option component, and an additional space located in the front of the enclosed infant room. The toddler option room is in a physically separate space from the enclosed infant room. The toddler room by itself has enough square footage for 12 children.

The following square footage was calculated for the infant and toddler rooms together; 1071 sq ft divided by 35 equals 31 children. There is not enough square footage for the requested capacity of 46 children. Applicant plans to expand the infant space into a portion of the space that is currently designated for the preschool application. Applicant understands this will reduce the requested capacity of the preschool application. There is a separate nap area with cribs and cots for children no longer requiring the use of a crib. The crib area needs to be rearranged so cribs are not blocking exits and so staff can safely move the cribs and cots. The classrooms are still being set up and there is still construction in process. LPA Schnell reminded applicant that rooms will need to be cleaned, organized and equipped with toys and equipment for the infants and toddler option children, fire extinguisher, smoke and carbon monoxide detectors, cubbies for infants personal belongings. LPA Schnell observed changing tables applicant will ensure at least one changing table is within arms reach of a sink. The changing tables and sinks are located outside of the classrooms where the bathrooms for the preschool children are located. Parents will provide disposable diapers and wipes for their infants. Food is provided by the parents. Food for each infant is stored in the kitchen area or refrigerator and foods will be properly labeled with the child's name and date. Once children begin to transition to table food, the program will provide 2 snacks daily and parents will provide lunch. The kitchen has a refrigerator, 2 tub sink, a microwave, an insta-pot. and storage for dry foods.

Applicant was reminded that cleaning products and other chemicals are not to be stored with foods. The children will use a combination of disposable plates and cups and washable utensils. Parents will provide bottles/nipples and any other food service items for their own individual infants and the staff will ensure these items are sent home with the infants each day. Napping equipment for older infant children developmentally too old for a crib will be a cot with sheets and blankets will be provided by parents and the napping equipment and bedding will be laundered daily by parents as well. Drinking water for the children will be provided by plastic baby water bottles or cups with lids provided by the parents. Medications for infants will be stored in the kitchen in high cabinets or in locked boxes in the refrigerators. The food service area and diaper changing area are located in separate areas of the building. All furnishing and equipment available to be observed today was in good condition and safe for use. There still needs to be garbage cans with tight fitting lids, and a first aid kit with all required components placed at the facility for review/observation. There are 5 sinks in the infant classroom for children's usage. All sinks accessible to children deliver water no warmer than 120 degrees. For older infants engage in potty training early, the staff will use potty chairs or the preschool bathroom, at separate times from the preschool program. There is a separate bathroom for staff. The isolation room for ill infant will be in the teachers office. Children will be signed in and out daily with electronic signature.

Outdoor Infants: The outdoor area for Infants measures 607.876 square feet allowing for 8 infants. Applicant needs to request a rotation waiver to ensure all infants are provided with outdoor time each day. Since there is not at least 50% available outdoor space for the requested capacity, LPA Schnell advised applicant to obtain a permit for using a public park nearby through San Francisco City Parks and Recreation Department and apply for an open space waiver with Community Care Licensing once the waiver is obtained. LPA Schnell will email the applicant more details on the requirements for the waiver. The outdoor area surface and equipment is observed to be clean, in good condition, and age appropriate. Construction materials needs to be removed from the outdoor space. There are no climbing structures. Shade needs to be provided for the outdoor space. Drinking water will be made accessible to children by water bottles/cups with lids provided by the parents.

The toddler option room is physically separate from the infant room. The toddler option program will share the bathroom with the preschool children at separate times. The room was observed to have no health and safety hazards, but needs to be compelely set up with todays and equipment for children to use. Licensee understands that the age range for the toddler option is 18 months to 3 years old and that authorized representatives must give written permission for their child to participate in the toddler option component as well as for their child(ren) to move out of the toddler option room before the age of 3.

Licensee understands that the toddler option children must be kept physically separate from the preschool age children. Ratio and staffing for the toddler option were discussed.
The following items were reviewed as part of today's visit: Record keeping for staff, children and facility records, maintaining and updating needs and service plans for infants, Care and Supervision of the Children, Child Discipline Procedures- talking and redirection will be used, Emergency Evacuation Procedures, Medication Policies, Isolation of Sick Children, Napping Requirements (Safe Sleep and SIDS discussed), Completion of Individual Infant Sleeping Plans and supervision of napping infants, Food Service for Infants, Transportation-none provided, Parents Rights, and Reporting Requirements. Applicant will ensure all require postings are displayed and visible to children’s authorized representatives at all times. The requirement for the testing of lead in the water was discussed (H&S Code 1597.16). LPA informed the applicant to review Provider Information Notice 21-21-CCP (dated 7/28/21) for directives and regulations regarding obtaining a test of the water for lead, and how that information is to be documented and reported to Community Care Licensing.

The facility will conduct monthly disaster drills and a log will be maintained for review upon request.
The fire inspection has not been conducted yet since construction is still in process.

This facility will provide Incidental Medical Services – IMS. LPA reviewed storage area for medication and equipment/supplies, and reviewed forms that will be used. LPA Schnell reviewed the COVID-19 self-assessment guide and required postings with the applicant today. Applicant was informed that access to available Personal Protective Equipment (PPE) may be available through the local child care resource and referral agency.

A follow-up pre-licensing inspection will need to be conducted to completely evaluate the physical plant once construction is finished and the facility is completely set up.

Applicant will need correct items identified in this report before a license can be recommended. Also, and updated Parent Handbook needs to be submitted. Updated LIC 200A, LIC 999 and LIC 401 forms will need to be completed to reflect the actual allowable capacity based on square footage.
