On September 2, 2021 at 3:00pm, Licensing Program Analyst (LPA) Catrina Quimbo met with applicant/director, Susan Osterhoff, for a scheduled, pre-licensing inspection. This is a change of ownership application (Facility previously licensed #384002426). Facility is currently operating through change of ownership process. Applicant/director understands applicant’s liability responsibility during change of ownership process. The applicant is requesting a preschool license for 12 children (ages 2 to entry into first grade). The facility will operate Monday through Friday 8:00am – 3:00pm. The facility was inspected, indoors and outdoors, for health and safety hazards and measured to calculate capacity.

Indoor: There is one classroom that will be used for the preschool program. Classroom #1 square footage was calculated for 415.6 sq. ft allowing for a capacity of 12 children. LPA observed indoor space for children to be clean and all furnishing, toys and equipment observed to be safe, age appropriate and in good condition. Sufficient store space for children’s personal belongings are located outside the classroom, labeled with each child’s name. Cots are to be utilized for napping children. Facility will provide sheets and blankets for napping children. Laundering and sanitization was discussed with applicant. Facility will launder sheets weekly or as needed. Classroom is equipped with a fire extinguisher, smoke and carbon monoxide detector and professional fire alarm throughout the building. First Aid Kit and emergency supplies are located in the classroom, inaccessible to children. Garbage cans have tight fitting lids. The facility is adequately ventilated and free of insects and bugs. Cleaning products are stored in facility’s high cabinets, inaccessible to children.

Indoor space also includes a “Gross Motor/ Multipurpose Area”. Multipurpose area is to be utilized as additional activity space and was calculated for 1,544.9 sq. ft, allowing for a capacity of 44 students. Due to space being a multipurpose area and not used as a regular classroom on a regular basis, this space is not included in total square footage/capacity.

There is one bathroom outside the classroom, located next to children’s storage area. Children’s bathroom includes 1 sink and 1 toilet. Total allowable capacity for toilets and sinks is 15 children. Bathroom also includes a changing table within arm’s reach to sink. Accessible sink to children does not produce water that is warmer than 120 degrees. LPA observed bathroom to have age appropriate equipment that is clean and in good working condition.

Facility also contains off-limit kitchen area, staff bathroom and staff office. Each off-limit area is made inaccessible to children through child safety gates and/or locked doors. For food service, facility will provide dry snacks while parents provide lunch. Facility will provide reusable children’s plates and utensils and will sanitize items through dishwasher in off-limit area. Allergies, food preparation, food storage and food service was discussed with applicant.

Medications with children’s individual names and required documentation will be stored in facility’s high cabinets, inaccessible to children. This facility will provide Incidental Medical Services (IMS). Outdoor: The outdoor area measures 625.68 sq ft., allowing for a capacity of 8 children. Applicant to request a waiver and rotation schedule due to lack of square footage space for requested capacity of 12 children. The outdoor area is enclosed by an at least four-foot high fence. Children are to bring their own water bottles. Facility has accessible refillable water station if needed.

During inspection, the following items were reviewed and discussed: care and supervision of children, child discipline procedures-staff will use communication and redirection, medication policies, isolation of sick children, napping requirements and sanitization, food service, and reporting requirements. Required postings were observed during today’s inspection.

An electronic copy of this 3 page report was emailed to applicant, susan@projectcommotion.org. Applicant was advised to acknowledge report was received, reviewed and is correct.

Prior to recommended licensure, applicant must complete following:

-Fire clearance
-Request for outdoor waiver
-Original license of ICRI-Project Commotion-Las Luciernagas with written statement stating license is closed due to change of ownership.
-Transfer all staff members cleared fingerprints from Fac# 384002426 to Fac# 384004459.
