On August 27, 2021 at 9:00am, Licensing Program Analyst (LPA) Catrina Quimbo met with applicants Lindsey Barnes and Yeimmi Alejandra Tryon for a scheduled, pre-licensing inspection. The applicants are requesting a combination center of a preschool license with a toddler option. The applicants are requesting 27 preschool children (ages 3 years to entry into first grade) with a toddler option of 15 children (ages 18 months to 36 months). The facility will operate Monday through Thursday 8:00am – 5:30pm, and Fridays 9:00am – 4:00pm. Children will be signed in and out with an electronic signature. The designated director/ for the facility is Yeimmi Alejandra Tryon. Facility’s main entrance is located on 4th Street, Mission Creek Park street side. The facility was inspected, indoors and outdoors, for health and safety hazards and measured to calculate capacity.

Indoor: There are two classrooms that will be used for the preschool program with the toddler option. Preschool classroom (#1) square footage was calculated for 1,032.5 sq. ft, allowing for a capacity of 29 students. The toddler classroom (#2) square footage was calculated for 510.488 sq. ft, allowing for a capacity of 14 students.

The facility contains additional indoor space of a multipurpose area and “quiet room”. The “quiet room” is used as an isolation area for sick children. Staff bathroom will be used for sick children. Applicants were advised children using the “quiet room” and staff bathrooms must always be supervised. The multipurpose room is to be utilized as additional activity space and was calculated for 375.12 sq.ft, allowing for a capacity of 10 students. Due to space being a multipurpose area and not used on a regular basis, this space is not included in total square footage/ capacity.

LPA observed indoor space for children to be clean and all furnishing, toys and equipment observed to be safe, age appropriate and in good condition. Sufficient store space for children’s personal belongings are located inside each classroom. Mats are to be utilized for napping children. Facility will provide sheets and blankets for napping children. Laundering and sanitization was discussed with applicants. Facility will launder sheets weekly or as needed. Each classroom is equipped with a fire extinguisher, smoke and carbon monoxide detector and 1 accessible sink for children. First Aid Kit and emergency supplies are located in the classroom, inaccessible to children. Garbage cans have tight fitting lids. The facility is adequately ventilated and free of insects and bugs. Window screens are new and in good condition. Cleaning products are stored in the classroom’s high cabinets, inaccessible to children.

Bathrooms are located inside the classrooms. Bathroom #1 (located in between Preschool and Toddler classroom) has 2 sinks and 2 toilets. Bathroom #1’s total allowable capacity for toilets is 30 children and total allowable capacity for sinks is 30 children. Bathroom #2 (located in Toddler classroom) has 2 sinks and 1 toilet. Bathroom #2’s total allowable capacity for toilets is 15 children and total allowable sinks is 30 children. All sinks accessible to children provide water that is not warmer than 120 degrees.

Each classroom is equipped with an adult sink, refrigerator, and dishwasher. Toddler classroom is also equipped with a 1 washer and 1 dryer. For food service, facility will use a food catering program. Food is to be catered to facility and be prepared in each classroom separately. Facility will provide reusable children’s plates and utensils and will sanitize items by separated dishwashers. Children are to bring their own bottled water. Drinking fountain is available to children if needed. Allergies, food preparation and food service was discussed.

Medications with children’s individual names and required documentation will be stored in classroom’s high cabinets. Medications that require refrigeration will be stored in classroom’s refrigerator. This facility will provide Incidental Medical Services (IMS). Outdoor: The outdoor area measures 783.158 sq. ft allowing for a capacity of 10 children. Applicants to request a waiver and rotation schedule due to lack of square footage space for requested capacity of 42. The outdoor area includes a drinking fountain available for children in care. An at least four-foot high fence encloses the outdoor area to be used. Adequate shade is provided by designated tarps hanging overhead.

The following items were reviewed as part of today’s visit: Record keeping for staff, children and facility records, Care and Supervision of the Children, Child Discipline Procedures-staff will use redirection, Emergency Evacuation Procedures, Medication Policies, Isolation of Sick Children, Napping Requirements, Food Service, Parents Rights and Reporting Requirements. Required postings were observed during today’s inspection. Facility will conduct monthly disaster drills and a log will be maintained for review upon request. The requirement for Lead Water Testing was discussed (H&S Code 1597.16).

During inspection, LPA received: corrected Application form, Criminal Record Statement, Health Screening reports, Personnel Policies, Job Descriptions, and Mandated Reporter training for Yeimmi Alejandra Tryon.

An electronic copy of this 3 page report was emailed to hello@thedahliaschoolsf.org, on August 27, 2021. Applicants were asked to review the report and return a signed copy of the report to LPA Quimbo.

Prior to recommended licensure, applicants must complete following:

-Fire clearance
-Request for outdoor waiver
-Written statement of no ownership and/or affiliation to other licensed facilities.
-Covering pothole in outdoor area
-Sensor alarm installations for side gate in outdoor area and double exit doors in preschool classroom.
-Proof of completion for at least 1 director or teacher with 16 hours of health and safety training (8 hours in EMSA certified CPR and First Aid Training; 8 hours in EMSA certified Preventative Health and Safety Training)
