On 08/12/2021, at 11:45am Licensing Program Analyst (LPA) Marissa Soto conducted an on-site visit and met with Director, Melinda Gomez. Facility is still open during the COVID-19 pandemic, and todays census consisted of 1 child supervised by 1 staff member. The Director took LPA on tour of the classrooms. The purpose of the visit is to inform the Director of the findings for the above complaint allegation. It was alleged Staff failed to use safety measures to prevent spread of communicable diseases and the Facility is not adhering to the terms and conditions of the admission agreement. During the investigation, LPA toured, made observations, reviewed records, and conducted interviews. Interviews conducted did not provide any corroboration or evidence to support the allegations. LPA observed staff using safety measures to prevent spread of communicable disease. LPA also observed documentation to parents and also incident reports. LPA learned that safety measure was taken to prevent spread of disease and also the facility is adhering to the terms of conditions of the admission agreement.
Based on the information gathered throughout the course of the investigation there was not a preponderance of evidence to prove or dismiss the allegation. This complaint has been determined to be UNSUBSTANTIATED: meaning that although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the alleged violations occurred.

Exit interview conducted and appeal rights were discussed. A copy of this report, Notice of Site Visit, and appeal rights were provided to the Directo. Notice of Site Visit is posted for 30 days.
