25 | During today's inspection Licensing Program Analysts (LPAs) Christopher Jackson and Charlotte Baney met with Director, Colleen Goncalves to follow up on an Unusual Incident Report (UIR) submitted to Community Care Licensing on 09/19/19. During today's inspection the facility was toured. Present were 104 children in care and 12 staff.
LPAs interviewed the Director, regarding the incident. LPAs reviewed and discussed this report with the Director. The facility reported the UIR to Community Care Licensing within 24 hours. A written UIR was submitted within 7 days, describing the specifics of the incident. On 09/19/19 one child was found in the enclosed hall area where parents wait for pick up. The director stated the child's is usually one of the first to be picked up. The child was trying to meet up with their parent in the hall area, where the child found their parent in the line. It was stated the child was able to walk out of the classroom and into the hallway without staff supervision. The following items were conducted to ensure the health and safety of children enrolled. The facility conducted a staff meeting on 09/19/19 which covered ratios. The facility will have two staff remain in the rooms during all dismissal times. Classroom dismissal: children shall remain on the carpet area, behind the half door till the parent enters the classroom and the child is called for, to be released to the parent. The director reiterated the staff will stand in front of the half door to ensure visual supervision of the children as they leave.
The following Title 22 Deficiency is being cited on the subsequent 809-D page. Upon receipt of Type A citations, licensee shall post and provide copies of the LIC 809-D for parents/guardians of children in care and for parents/guardians of newly enrolled children for the next 12 months. Licensee must also keep the signed LIC 9224, acknowledging receipt of LIC 809-D in each child's file. Appeal Rights and Notice of Site Visit were provided. Notice of Site Visit must remain posted for 30 days. |
