25 | Licensing Program Analyst (LPA) Mariya Melnichuk and Licensing Program Manager (LPM) Bettina Engelman met with Director, Christina Hines on 1/23/2023 at 2PM for the purpose of an unannounced plan of correction inspection to clear a Type A deficiency, which was issued on 1/11/23 for co-mingling toddlers with preschoolers. Two type B deficiencies were also issued during the visit. The facility did not have parent/guardians sign in and out children with a signature. The facility was also cited for not documenting 15 minute sleeping logs for children under 24 months of age.
All individuals subject to criminal background review have obtained a criminal record clearance. Director was reminded that all adults 18 years and older, working in the center, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
During today's inspection LPA toured all areas accessible to children in care and observed sixteen preschoolers, and eight toddlers, being supervised by the teachers. The toddlers were taking a nap in a room, separate from the preschool-age group. Director acknowledged that she must comply with her license terms and conditions. She has provided training on toddler option and Safe Sleep Regulations to her staff since the last visit. At this time, there are no toddlers under 24 months enrolled. Director stated that the board decided to discontinue the toddler component, and submitted an updated application to remove toddler option during today's inspection. LPA viewed the sign in/out binder and verified that parents provided signatures upon arrival and departure of the child. LPA reviewed the LIC 9224 acknowledgment forms signed by parents.
All deficiencies cited on 1/11/23 are cleared, effective today. Proof of correction letter was provided. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
