13 | On 08/02/2021, at 11:00am Licensing Program Analyst (LPA) Marissa Soto conducted an on site visit and met with Family Service Worker, Nohemi De Alba-Vega. Facility is still open during the COVID-19 pandemic, and todays census consisted of 13 children supervised by 4 Staff members. Nohemi took LPA on tour of the classroom. The purpose of the visit was to inform Nohemi of the findings for the above complaint allegation. It was alleged a Daycare Child sustained unexplained injury while in care. During the investigation, LPA toured, made observations, and interviews were conducted with staff, an adult, and a child. Interviews conducted did not provide any corroboration or evidence to support the allegation. LPA observed staff adequately supervising children and documentation of incident reports in children's files. LPA learned that staff notify families of child injuries and that they are taking preventative measures to guide children's behaviors. LPA discussed personal rights with the Family Service Worker.
Based on the information gathered throughout the course of the investigation there was not a preponderance of evidence to prove or dismiss the allegation. This complaint has been determined to be UNSUBSTANTIATED: meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation occurred.
Exit interview conducted and appeal rights were discussed. A copy of this report, Notice of Site Visit, and appeal rights were provided to the Family Service Worker. Notice of Site Visit is posted for 30 days. |
