1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 11/29/2023 at 9:30 AM, Licensing Program Analyst (LPA) Tiffanie Diep and Licensing Program Manager (LPM) Chayntel Hunter met with Center Manager Jenny Almodovar for the purpose of an unannounced annual inspection. Entrance Checklist (LIC 125) was provided to Center Manager and Center Manager guided LPA and LPM on a tour of the facility. LPA observed five staff supervising 32 children. Facility days and hours of operation are Monday through Friday from 8:30 AM to 3:00 PM. LPA verified that annual fees are current. All individuals subject to a criminal record review have obtained a criminal record clearance. Center Manager was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of five days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. A health and safety inspection of the facility’s interior and exterior was conducted. LPA reviewed the sign-in/out sheets and observed that children were properly signed in/out. LPA observed the required postings. LPA reviewed staffing ratios, first aid supplies, furniture, equipment, fire drills, and access to drinking water. LPA verified that hazardous items (disinfectants, cleaning solutions, etc.) were inaccessible to children in care. LPA observed both smoke and carbon monoxide detectors were functional. LPA observed adequate age-appropriate toys and equipment available for children. Facility provides breakfast, snack, and lunch. The outdoor play area was inspected. LPA observed the play structure was in good repair and was sufficiently cushioned by foam. Continues on 809-C

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every five years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

LPA reviewed children’s files and verified each child’s file contained required documents, including emergency information. LPA reviewed staff’s files and observed required documents, including health screening reports with TB clearance and required immunizations. LPA discussed the mandated reporter training requirement and Center Manager was reminded to renew the course every two years. LPA verified at least one staff member present has current EMSA pediatric CPR and first aid certification which expires on 05/20/2025.

This facility provides Incidental Medical Services - IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Center Manager was informed of the MyChildCarePlan.org website, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Based on LPA’s observations, interviews conducted, and records reviewed, a deficiency is being cited on the attached LIC 809-D. LPA Tiffanie Diep informed the center manager, Jenny Almodovar, that this report dated 11/29/2023 documents one Type B citation as there was a potential risk to the health, safety, or personal rights of children in care.

An exit interview was conducted and report was reviewed with the center manager, Jenny Almodovar. A notice of site visit was given to Center Manager and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
