Licensing Program Analyst (LPA) Lauren Scott met with Director, Carmen Baugh to open and close the complaint investigation regarding the above allegation.

On 5/2/2023 CCLD received a UIR from facility stating a child had been left outside of the classroom for approximately 5 minutes. LPA conducted a formal interview with director regarding incident as well.

Based on the review of records and interviews conducted, it was revealed that there was an absence of supervision of at least one child. Therefore, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. Title 22 regulations are being cited on the attached 9099-D page.

An exit interview was conducted with the Director, Carmen Baugh. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

LPA Scott informed director, Carmen Baugh that this report dated 5/3/23 documents one Type A citations which shall be posted for 30 consecutive days as there is an immediate risks to the health, safety, or personal rights of children in care.

Also, LPA Scott informed the Director, Carmen Baugh to provide a copy of this licensing report dated 5/3/23 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
