Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) Alecia Sifuentes and Licensing Program Analyst (LPA) Janie Davis met with Director Carmen Baugh for the purpose of a case management inspection. Licensee previously requested an increase in capacity for 87 children, however the Licensee has requested to change the increase in capacity for 90 children ages two years to entry into first grade. The fire clearance (STD850) was granted and received on 8/22/2024, however LPA will submit an amended STD850 for approval. The program currently operates Monday through Friday from 6:30 a.m. to 6:00 p.m.

INDOOR ACTIVITY SPACE:
There are eleven classrooms in Rooms #2, #3, #4, and #5 located downstairs and Rooms #21, #22, #23, #24, #25, #26, and #27 located upstairs. LPA observed a sufficient amount of equipment, toys, tables, chairs, cubbies, and napping cots. LPA used current measurements from report dated 8/9/2018, therefore LPA did not take measurements for Rooms #2, #3, #4, #5, #21, #22, #23, and #24. LPA measured Rooms #25, #26, and #27. The new total classroom spaces contain a total of 3,170 square feet, which will accommodate Licensee’s request for 90 children. There are seven toilets/one urinal and eight sinks located upstairs and downstairs for the children. There is a separate staff restroom located in another section of the church building. Individual measurements are recorded on the Capacity Worksheet (LIC 9024).

OUTDOOR ACTIVITY SPACE:
There is one outdoor area on the property. LPA used current measurements from report dated 7/20/2017, therefore LPA did not take measurements. The outdoor activity space contains a total of 24,792 square feet, which will accommodate Licensee’s request for 90 children.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview conducted and report was reviewed with Director Carmen Baugh.

The follow items are required before the change of capacity will be approved:

1. Child Care Center Applications Orientation (part 1) Certificate and Child Care Center Operations & Record Keeping Orientation (part 2) Certificate for Licensee Representative, Dan Mahaffie.
2. Amended Fire Inspection Request (STD850) reflecting 90 children.
