On 09/02/2021 @ 10:20 AM, Licensing Program Analyst (LPA) Francisca Velazquez conducted an unannounced Annual/Required inspection of the Child Care Center (CCC). Prior to entering the facility, LPA check in at the school office to conduct COVID-19 pre-screening questionnaire. Based on staff responses, it was determined that facility was safe and free of any COVID-19 exposures. LPA met with Sunshine Frawley Lead Preschool Teacher and Jessica Lopez Preschool Aide and explained the purpose of the inspection. LPA, in the company of the Lead Preschool Teacher toured the interior and exterior of the center. The center operates from 9:15 AM to 12:15 PM on Monday, and 8:15 AM- 12:15 PM Tuesday- Friday. The center uses one classroom for care and had a population of 16 children in care at the time of the inspection.

The classroom of the center is clean and organized. The classroom has age appropriate furnishings available for children. The classroom's carbon monoxide detector was tested and found to be inoperable at 10:48 AM. Lead Preschool Teacher was reminded of the necessity to have a functioning carbon monoxide detector on site. The restroom is in safe and sanitary operating conditions. The bathroom contains a single toilet and a sink. Children also have access to restroom at the school site by outdoor yard. Lead Preschool Teacher stated that children rarely use that restroom. When children are outdoors and need to use the restroom, one teaching staff will walk child inside to use the restroom.

Disinfectants, cleaning solutions and sharps are secured in an elevated cabinet in the classroom.

The center provides snacks for the children provided by school district. LPA observed a posted menu in the facility. Likewise, the classroom has licensing required notices posted near the classroom entry/exit. The LPA reviewed the center Sign In/Out forms which were completed appropriately. All children present during inspection were signed in appropriately.

The center’s outdoor playground area is completely fenced and has plenty of tables and gross motor equipment to allow children to social distance. LPA observed age appropriate furnishing in the play area.

The play structure in the play area has adequate cushioning materials (sand) to absorb falls and an abundance of shade (i.e. tress). No bodies of water were observed on site. There is available drinking water for children both inside of the classroom and outside in the play area by means of water bottles. Waste receptacles with tight fitting covers were observed in the center.

A sampling of staff and children records were reviewed. The children’s records were complete and found to contain emergency contact information as well as medical consent form. Staff CPR/First aid were reviewed and found to be current and expires 7/20/23. Lead Preschool Teacher stated no firearms/weapons or ammunition was stored on the premise.



This facility does provide Incidental Medical Services (IMS). Lead Preschool teacher stated that all health care needs are provided by the school’s nurse. During this inspections there were no deficiency cited.
