25 | Licensing Program Analysts (LPAs) Marie Rodriguez and Jyoti Saini made an unannounced Case Management visit to follow up on a self reported unusual incident that was reported to CCLD office by phone on January 27, 2020. LPAs met with Director Rosie Morales and explained purpose of visit. LPAs toured physical plant, reviewed pertinent documents, and conducted interviews.
On January 24, 2020, Staff S1 and S2's class had just finished eating lunch in the cafeteria when they had their children line up at the door to return to their classroom. S1 and S2 did not realize Child C1 had stayed behind in the cafeteria. Staff S3 and S4's class were still eating lunch in the cafeteria near the door at that time. S3 realized after going to the kitchen that C1 was by the door of the cafeteria and had informed S4. S4 went to C1 and brought C1 back to where their class was sitting. S3 and S4 brought C1 with them after lunch was over to the bathroom for a bathroom break. S2 went to look for C1 and found C1 with S3 and S4's class in the bathroom. S2 brought C1 back to their classroom.
This appears to be an isolated incident. In response, Facility has provided training on proper supervision to all staff during the staff meeting on January 28, 2020. A specific training with the staff from the classrooms involved was given on January 29, 2020. A training for all preschool classes was given on January 30, 2020. Teachers involved were spoken to about the incident and will have one on one coaching with the director for four weeks to help with classroom management and transitions. Facility took appropriate action.
No deficiencies cited today.
Report was reviewed and discussed with Director Rosie Morales. A copy of report was provided.
Notice of site visit was observed being posted and shall remain posted for 30 days. |
