25 | On July 27, 2023 at 1:30pm, Licensing Program Manager (LPM), Garfield Leung and Licensing Program Analyst (LPA), Catrina Quimbo, met with associate director, Samaa Srouji, and elementary director of student services, Melanie Cabie, for an informal office meeting.
The purpose of the meeting was to discuss the self reported unusual incident that occurred May 8, 2023. 2 teaching staff and 5 enrolled toddlers exited facility from back door of Toddler Unit classroom to walk to outdoor playground. One of the enrolled toddlers (19 month old) was able to walk away from the group, in the opposite direction from teaching staff. Toddler walked towards facility's open gate to parking lot. Toddler did not enter the parking lot.
Toddler was found by a teaching staff member whom works in Highland Elementary school. Toddler was brought back to preschool program. Per director, toddler was left unattended for approximately 2 minutes. Per associate director, entire incident of toddler leaving group, being found and returning to group was approximately 5 minutes. A case management by LPA was conducted May 24, 2023.
LPM Leung discussed the citation and civil penalty issued for incident. Plan of correction are as follows:
- Director and associate director immediately informed toddler's parents of incident.
- Director and associate director met with teaching staff individually, discussing incident.
- Director held a mandatory staff meeting for all preschool staff on 5/11/2023, reinforcing the importance of supervision and head count protocols.
- Staff will ensure gate to parking lot will always be closed prior to children's use of outdoor playground.
- Pickup policy changes for children's families during outdoor play time. Families have options to wait for their child to return to classroom, going to preschool office alerting personnel staff of pick up, or families directly messaging preschool staff.
- Walking children through the preschool building for outdoor play time instead of exiting through classrooms' back doors.
- Staff will maintain communication with both children and staff.
Plan of correction issued during case management has been cleared with today’s office meeting.
