On November 8, 2022 at approximately 8:15am, Licensing Program Analyst (LPA) Catrina Quimbo conducted an unannounced, complaint visit to Bright Horizons @ Gilead (Infant) center. LPA met with health and safety director, Roxanne Magadia, and center director, Michelle Lee, and explained the purpose of the visit.

At the start of LPA's visit was drop off time with staff and children arriving to facility. Facility is licensed to operate out of 6 classrooms. Under infant license, there are 2 classrooms: Starfish and Seahorse. At approximately 9:30am, there were 5 children with 3 staff present in Starfish classroom and 7 children with 4 staff in Seahorse classroom.

As part of this investigation, LPA conducted classroom observsations, reviewed program's records and documents, interviewed random selection of staff and interviewed random selection of enrolled children's parents.

LPA conducted unannounced, classroom observations in Seahorse classroom. During classroom observations, LPA observed staff to provide day care children with sufficient care and supervision, observed staff changing multiple enrolled children's diapers and observed staff to maintain children's hygiene needs by washing hands and sanitizing the classroom. Random selection of staff interviewed stated children are always supervised with sufficient care. Random selection of parents interviewed stated they have no concerns over level of care and supervision staff provide to their children.

Per facility's Parent Handbook, infants and toddlers can bite for a variety of reasons and program will respond to incidents with a plan of action. Staff stated program will work with children's families on recognizing techniques, to address children biting. Staff have also provided alternate plans/solutions to meet all children's needs.

Per staff interviewed, program's protocols of a child receiving an injury includes notifying child's parents through a phone conversation and/or written injury reports. Random selection of parents interviewed stated they have been informed by staff of their children receiving an injury through a phone conversation and/or written injury report.

Staff stated children's diapers are changed at least every 2 hours or as needed. Facility records when a child's diaper is changed and are changed in accordance with requests of families. Parents interviewed stated they are always notified by staff of their child's bathroom and diapering habits.

Facility's Parent Handbook states that, although staff will take precautions to ensure children's belongings are handled with care, center is not responsible for damaged clothing. Staff stated enrolled children's clothes are changed as needed. Random selection of parents interviewed stated they do not have any concerns of facility meeting hygiene needs.

Although the above allegation may have happened or is valid, based on LPA's classroom observations, interviews, and record review which was conducted, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview conducted and report was reviewed with center director, Michelle Lee.
