On July 30, 2024 at approximately 8:50am, Licensing Program Analyst (LPA) Catrina Quimbo conducted an unannounced, annual inspection. LPA met with licensee, Zhanneta Karapetyan, and explained the purpose of the visit. Present during visit included licensee, teacher assistant (T1), and 8 children (1 infant and 7 preschool age). Licensee is operating within capacity limits and ratio.

Licensee lives in the home with their adult daughter, adult daughter's spouse, and minor grandchild. All adults living and/or working in the home have fingerprint clearance on file.

Hours of operation are Monday through Friday 7:30am to 5:30pm. Licensee operates home as a Montessori program. With licensee, LPA inspected day care areas, indoors and outdoors, for health and safety hazards. The DAY CARE AREAS are the living room (main day care area), dining area, hallway bathroom, and backyard. The OFF LIMIT AREAS are the garage, office room (next to garage), kitchen, and all bedrooms on upper level of home. There are stairs in the home that have a child safety gate installed.

Home is in good repair with proper temperature and ventilation. There are a variety of Montessori toys and materials that are in good condition. Flooring in home includes carpets that was observed to be clean and free of stains. LPA observed furniture for children's use to be age appropriate and in good condition.

Home is equipped with a fire extinguisher, first aid kit and multiple dual smoke/carbon monoxide detectors. LPA observed cleaning solutions and chemicals to be stored in an off limit area and/or on home's high shelves. LPA observed cabinets and drawers in day care areas to not have any accessible poisons or cleaning solutions.

Restroom for children's use was observed to be in working condition. Restroom includes appropriate toileting equipment. LPA observed a garbage bin with a tight fitting lid. Per licensee, children are brought to and from the restroom and are always supervised.

Children sleep on floor mats. LPA observed floor mats and sheets to be appropriately stored. Per licensee, infant present also sleeps on a floor mat. LPA did not observe a crib on site. Licensee stated they are aware children sleeping must always be supervised. LPA reminded licensee sleeping logs must be documented and maintained for napping infants in care.

Backyard is entirely fenced and enclosed. Backyard includes a variety of toys, materials and equipment that are in good condition. Flooring in backyard includes grass and artificial turf that is in good condition. LPA did not observe any pools, spas or bodies of water on site.

Licensee provides a food service to children that includes breakfast, lunch and snack. This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. Licensee has licensing documents posted and available for review. Licensee maintains a child care roster that was made available for review. LPA reviewed children's records and observed records to be complete. Licensee's CPR/First Aid certification is current and will expire 08/2024. LPA observed T1's CPR/First Aid certification to also be current and will expire 08/2024. Both licensee and T1 have required immunizations available for review. LPA observed licensee's Mandated Reporter certification to have expired 05/2024. LPA reminded licensee Mandated Reporter certification must be renewed every two years by all staff working with children. Per licensee, there are no weapons or firearms in the home.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their
existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and
discussed the Child Care Licensing Safe Sleep webpage at
https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the licensee, Zhanneta Karapetyan, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

No deficiencies were issued today during LPA's visit. A notice of site visit was given and must remain posted for 30 days.
