25 | On July 3rd 2024, Licensing Program Analysts (LPAs) Kassandra Medrano and Diana Alvarado met with Jianna Fontanilla, Site Supervisor, and Katia Alvarez, Senior child development director for an unannounced required –annual inspection. As well as to discuss an incident that occurred on 6/17/24 and was self reported by the facility on 6/24/24 During visit LPAs gathered pertinent information, and made classrooms observations. LPAs to return at a later date.. At this time, more information is needed. LPAs toured the indoor and outdoor areas of the facility during today's inspection. LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC 610), Earthquake Preparedness Checklist (LIC 9148), Parents' Rights Poster (PUB 393), Personal Rights (LIC 613A), Child Car Seat Law (PUB 269), Chefable menus, and Activity Schedule. Days and hours of operation are Monday through Friday: 7:30am-6pm. The preschool age program is Licensed in Rooms Monkey Koala, Otter, Dolphins, and Pandas. On site the licensee has another license, Gateway Child Care Center- Infants (414001720). The following was observed today: During review of criminal record clearances of staff it was found that there were 2 individuals that were not associated to the facility. LPA reviewed ten children's and 7 staff files during today's inspection. During review, it was observed that 7/8 of staff are missing documentation of current flu shot or declination. It was also observed that staff were missing mandated reporter training. The staff files that were reviewed have current CPR and First Aid certifications on file. Facility representative understands that there shall be at least one person, with valid CPR and First Aid certifications, on site at all times or present during off-site activities (field trips). During review off children’s files it was observed that children’s files are complete and up to date. LPA observed that the teacher/child ratio was in compliance during today's inspection. Facility representative understands the conditions, limitations, and capacity specifications of the facility license. Last fire/disaster drill was completed on 6/25/2024. Facility Representative understands that children shall be always visually supervised. Any child(ren) who exhibit symptoms of illness including, but not limited to, fever or vomiting, are not accepted in care. Facility Representative stated that when and if a child becomes ill they isolate the child in the library until parents pick up the child. This facility provides Incidental Medical Services – IMS.
