On March 3, 2020, at 1:50 pm, Licensing Program Analyst (LPA) Van met with the Administrator, Shaylin Montgomery, for an unannounced annual inspection. The purpose of the inspection was explained and was granted entry to the facility by the Admin. This is a preschool program, present today are 14 teachers, 3 aides supervising 93 children. The facility is in compliance with the staff/children ratios today. Days and hours of operations for the preschool are from Monday - Friday 8:00 am – 6:00 pm. The Managing Director, Jennifer Piccinini, arrived during the inspection.

At 2:00 pm, LPA, along with the Admin, inspected all classrooms for health and safety hazards. The center is clean, safe, sanitary, and in good repair. LPA observed that each classroom has its sign-in / out record with legal signatures from the parent or authorized representative. There are age-appropriate toys and learning materials available for children. All furniture and equipment are in good condition, free of sharps, loose or pointed parts. LPA observed no pools, spas, or other bodies of water at the facility. All disinfectants, cleaning solutions, poisons, and other items that are dangerous to children are locked and stored inaccessible to children. The facility has a centralized smoke alarm, fully charged fire extinguisher, and a working telephone at the site; LPA observed the facility has a carbon monoxide installed in the hallway. The facility has a total of 8 toilets and 14 sinks, all toilets and sinks are in safe and sanitary operating conditions. All medications are centrally stored, maintained with the child’s name, and are appropriately dated. The outdoor playground area is in good repair. Play structures are free of any loose parts. All areas around or under high climbing equipment and slides are cushioned with material that absorbs falls. Per the Admin, the sandboxes areas are inspected daily before children play. The facility has posted the required forms (i.e., License, menus, waivers, Notification of Parent’s Rights, Notification of Personal Rights, Car Seat Law, and Emergency Disaster Plan).

Per the Admin, there are no firearms, weapons, or pets at the center; the center provides two snacks, and lunch is brought from home. Children brought their water bottle from home, and the staff will fill-up the water for the children throughout the days. The food preparation area is clean, organize, and foods store appropriately.

At 3:15 pm, LPA reviews the children’s and staff’s files, Care and Supervision of the Children, and Medication Policies. A review of staff records during today’s visit indicates that all staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Multiple staff have a record of valid Pediatric FirstAid/CPR card in file. Also, all staff have the required immunization and verification of the mandated child abuse reporting training as compliant with AB1207.

On December 4, 2019, annual inspection, LPA mentioned to the new Director, Amanda Thieviege, that she would need to submit Director paperwork and qualifications to licensing within ten days since she is a new director. During that annual inspection, LPA provided the new direction’s requirement checklist and also mentioned in the annual report; however, as of today, the Director has not sent any paperwork to licensing.

LPA reminded the lead teacher that as of September 1, 2016. A person may not be employed or volunteer at a childcare facility unless he or she has been immunized against influenza, pertussis, and measles.

LPA reviewed AB 1207 with the Managing Director. As of January 1, 2018, all staff is required to complete Mandated Reporter Training every two years. The training can be obtained online at www.mandatedreporterca.com.

ADA information was provided; US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to the publication. Pesticides training was also discussed (Information on the DPR website at www.cdpr.ca.gov/shoolipm/childcare). Incidental Medical Services (IMS) Plan was also discussed with the Managing Director.

During the inspection, LPA obtained an up today Emergency Disaster plan. The Managing Director states a new personnel report (LIC 500), and the Designation of Administrative Responsibility (LIC 308) will be sent to LPA. LPA also went over the Child Care Center checklist and provided a copy to the staff during the inspection for reference in the future inspection.

See 809D for deficiencies that are cited today. The copy of this report is reviewed and provided to the Director. Notice of site visit is posted and shall remain posted for the next 30 days.

This requirement is not met as evidenced by; Record review; LPA had not received any paperwork from the new Director. During the inspection on December 4, 2019, annual inspection, LPA also mentioned to the new Director to submit the required paperwork to update her status. However, LPA has not received any documentation from the Director; this poses potential health and safety risks to children in care.
