25 | Licensing Program Analyst (LPA) Van conducted an unannounced case management inspection today and met with the Director, Leah Catching, in response to an unusual incident reported to licensing on August 26, 2021. The incident involved C1, who was left behind and unattended during a transition time.
Along with the Director, LPA evaluated the physical space where the incident occurred and interviewed teachers who were directly involved in the incident. Per Director, The incident happened on July 30, 2021. However, the same week, the facility had a COVID-19 exposure, and the facility had to close for 14 days as recommended by the Department of Public Health. Therefore, the Unusual Incident was not reported to Licensing promptly. Based on the interview and facility report, it was determined that a lack of care and supervision had occurred on July 30, 2021. C1 was not accounted for and left unsupervised outside in the play yard, standing in front of the closed door of the Arctic classroom. C1 was spotted by another teacher who was transitioning their classroom from inside to the play yard.
As a result of lack of supervision, the following Title 22 deficiency is cited on the 809D page. An exit interview was conducted; Plans of corrections were reviewed and developed with the Director. Appeal Rights was discussed with the Director. Director was informed that reports citing Type A violations are to be provided to parents/guardians of children currently enrolled and parents/guardians of children newly enrolled at the facility during the next 12 months. Parents/guardians must sign Form LIC9224 to be kept in each child's file. Today's report, 9/22/2021, and notice of site visit will be sent to the Director by email at lcatching@learninglinkspreschool.org by the close of business on 9/22/21. Confirmation of receipt is required. LPA informed the Director that the notice of site visit, Type A report, and deficiency page must be posted for 30 days. |
