25 | Licensing Program Analyst (LPA) Andrea Medlin met with facility representative for this case management visit. Purpose of the visit is due to a self reported unusual incident reported to licensing. On 10/23/2023, a child (C1) who is in the three year old classroom was outside on the playground during outside time. The child was able to go to an adjacent playground (two year old yard) without staff initially aware she was gone. The fence dividing the two playgrounds is tall enough that staff cannot see over the fence into the other yard. There is a gate with a latch that is out of reach of children, however per staff, the gate was not all the way closed and latched which enabled C1 to push through gate and go to other yard. LPA inspected outside yards and observed the playgrounds, fence, and gate with latch. The gate is supposed to be self closing, however LPA observed the gate does not all the way close as it should and properly latch close. C1 was left unsupervised for approximately 15 minutes. Due to this incident, facility has taken measures to try to reduce another incident including: modifications to the playground(s) including a different type of fence, or lowering of fence, so that teachers can have visual supervision between the areas more easily. Also, a new latch will be installed as well as ensuring gate self closes. Per area manager, Jennifer Piccinini, an all staff meeting will be held tonight on staff supervision. A copy of the meeting agenda and staff who attended will be sent to LPA.

This report was reviewed with facility representative, Janelle Johnston, and a copy of this report must be made available for public review upon request. Due to a Type A violation, this report and violation must be given to all current parents/guardians, and newly enrolled families for the next 12 months, and documented on the LIC 9224 and returned to each child's file. Notice of site visit posted and shall remain posted for 30 days. |
