On January 22, 2025, Licensing Program Analyst (LPA) Maria Olguin-Leon conducted an unannounced, annual inspection. LPA met with Director Mary William and explained the purpose of the inspection. Present in the facility today were, Director, 5 staff caring for a total of 16 infant children. At approximately 9:30am, LPA observed S5 to not have criminal background clearance, a type “A” deficiency was issued. Facility is operating within capacity and in compliance with staff/child ratio. Facility operates an infant and preschool program from Monday to Friday 7:45am to 5:45pm.

LPA and Director toured the indoors and outdoors of facility for health and safety hazards. Infant program operates in two classrooms; Baby Cubs classroom for infants ages are birth to 12 months and Mickey Mouse classroom for infants ages 12 months to 24 months. Classrooms are clean with proper temperature and ventilation. There are plenty of age-appropriate toys, infant furnishings, playmats, and learning materials all in good repair. Classrooms are equipped with storage cubbies for children to store personal items, diapers, and belongings. Classrooms are equipped with two diaper changing tables within arm’s reach of a sink. Tables are equipped with changing paper sheets, that are changed after each diapering. Potty chairs are available for potty training children. There is a separate bathroom in facility for staff use. All trash containers in classroom have tight fitted lids. LPA reminded Director to install child proof lock/latches to cabinets where cleaning products, chemicals and toxins are stored, technical violation was issued. infant sleeping area in baby cubs’ room is separate from infants’ activity area and has plenty of cribs with children’s names. Sleeping cots are available for the older infants and facility and parents provide sheets for cribs and cots and are washed weekly or sent home for washing.

Infants’ outdoor playground is separate from the preschool program. Playground is equipped with age-appropriate activity structures, push toys and other age-appropriate toys, LPA reminded Director to remove broken toys, all in good condition. Trees provide shade for children. Playground flooring is equipped with artificial grass to cushion falls. Playground is surrounded with a 5 ft wood fence and a 3 ft. fence to separate infant playground from off limits areas. Per Director, older infants use the preschool playground at scheduled times and do not comingle with preschoolers. LPA did not observe any pools, spas, or other bodies of water. Per Director, there are no weapons or firearms on the premises.

Infants’ food and bottles are properly labeled with child’s name and stored in classroom’s refrigerator. Kitchen preparation area in classroom was observed to be clean and food stored properly to prevent contamination. Parents provide children’s bottles and food for infants. Labeled water bottles with children’s names are brought from home for use indoors and outdoors. First aid kit is fully stocked with medical supplies. Incidental Medical Services were discussed. Sick children are isolated in classroom and away from other children.

Parents sign children in and out by scanning QR code via Brightwheel app. Parents use individual PIN #’s and signatures to sign in and out. LPA reviewed sign in/out on Brightwheel app. LPA observed fully charged fire extinguishers. Classrooms have a dual working carbon monoxide/smoke detector and a sprinkler system. Facility is equipped with a landline phone and staff use Brightwheel app. to communicate with facility staff and parents.

LPA reviewed 5 children’s files 5 staff files. All children and staff files are complete. One staff has CPR/First aid training. Children’s records included Safe sleep plan and infant needs/service plan. Sleeping logs are maintained on dry erase board, LPA reminded Director to maintain paper sleeping logs for all children under 24 months old, technical violation issued. Director was reminded to update LIC9227 when child can roll over, technical assistance provided. The facility has all necessary paperwork posted, Facility License, Notification of Parent's Rights, Notification of Personal Rights, Car Seat Law, Outdoor waiver, and Emergency Disaster Plan. Last emergency drill was conducted on November 22, 2024, and is properly documented.

LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. LPA reminded Director to update LIC9927 Infant safe sleep plan as soon as infants are observed to be rolling over continuously and to maintain paper sleeping logs for all infants under 24 months old and to keep these records for three years.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Director is aware that all staff is required to complete Mandated Reporter Training every two years. The training can be obtained online at www.mandatedreporterca.com. LPA encourages the director to frequently visit our website at www.ccld.ca.gov for licensing regulations and new updates.
