25 | THE FOLLOWING OFFICE MEETING WAS CONDUCTED VIRTUALLY VIA MICROSOFT TEAM..
Licensing Program Manager (LPM), Cindy Interiano, and Licensing Program Analyst (LPA), Leslit Tapia-Mandujano, met with Licensee, Amina Burrell. The purpose of meeting was explained and was to conduct an Informal Office Meeting regarding recent deficiencies cited on 06/24/2021.
Issues discussed during this meeting are:
· Staffing Ratio and Over capacity – Licensee’s Husband was caring for 9 children (1 infant and 8 Prek age children) upon LPA’s arrival at the facility on 06/29/21. Licensee arrived shortly after with 3 additional children (1 PreK and 2 School Age).
· Requirements for helpers and people living or working in the day care, including Helper’s requirement to have current CPR when left alone with children.
Capacity limits and requirements for a Small and Large license were explained. Capacity worksheet will be emailed to Licensee. Licensee was reminded that when operating at a Large capacity, there must be a Helper present. Licensee's children (under the age of 10 years old) will count in the capacity if they are present in the home.
Licensee submitted a letter of appeal on 06/25/2021, which is pending Regional Manager’s review. Licensee will be notified of the outcome at a later date. A Plan of corrections inspection was conducted on 06/29/2021. All deficiencies have been cleared. Licensee provided a written plan of corrections dated 07/07/2021, indicating a proposed plan on how to remain in compliance in the future.
Licensee was reminded to provide copy of the Evaluation Report and Type “A” Deficiencies cited, to the parents/guardians of children currently enrolled in care and to parents of newly enrolled children during the next 12 months. A signed and dated LIC 9224 shall be in all Children's files. Any repeat violations during the next 12 months will result in an immediate civil penalty.
Licensee states she understand the importance of being in compliance with the regulations. Licensee understands she will maintain the capacity of the license.
Large Family Child Care Home License is approved and will be effective as of today, 07/09/21.Fire clearance for a Large License was received on 06/21/21.
Report was read out loud during the virtual meeting. Report will be emailed to Licensee at AMINAECOWONDERSCHOOL@GMAIL.COM by end of business day today, 07/09/2021.
